210例2型糖尿病肾病相关性的分析

摘　　要：目的通过观察不同治疗方案下的DN患者尿白蛋白分泌率(UAER),探讨2型糖尿病肾病病因的相关性。方法根据两次测得的尿白蛋白排泄率(UAER),随机分为三组,每组70例。A组为常规治疗组,患者给予卡托普利降压治疗;B组为先口服卡托普利后胰岛素治疗3个月,再进行胰岛素治疗;C组为全程胰岛素治疗。3组患者均在糖尿病饮食治疗的基础上,不予低盐饮食。8个月内连续3次检测其尿白蛋白排泄率,并作好记录。结果组内比较,B组6个月后的UAER值、C组6个月后和8个月后的UAER值与自身最初值比较,差异有显著性,P<0.05。组间比较,6个月后的C组和8个月后的B组和C组的UAER值与最初值比较,差异有显著性,P<0.05。结论糖尿病肾病是糖尿病常见而严重的慢性并发症之一,在糖尿病微血管并发症的发生发展中也扮演着重要角色。尽早使用胰岛素治疗,能有效延迟糖尿病肾病的发生。Objective To explore the related etiology of type 2 diabetic nephropathy（ DN ） by observing the DN urinary albumin excretion rate（UAER）in different treatments of the disease. Methods Total 210 cases of DN were randomly divided into three groups（70 cases for each group）according to two-time consecutive measured UAER. Group A,conventional treatment group,was given nitrendipine antihypertensive therapy, Group B was given insulin therapy for three months after oral administration of nitrendipine, and Group C was given insulin therapy. The patients of the three groups were given diabetes diet therapy,without any low-salt diet. The UAER was detected and recorded three times within 8 months. Results There was a significant difference in the UAER value of Group B after six months compared with its own initial value,and in the UAER values of Group C after six months and eight months compared with its own initial value（P〈0.05）. There was a significant difference in the UAER value of Group C after six months and in the values of Groups B and C after eight months compared with their own initial values（P〈0.05）. Conclusion DN is one of the most common and severe chronic complications of diabetes and play an important role in the occurrence and development of diabetes microvascular complications. The early use of insulin therapy for diabetes can effectively delay the occurrence of DN.
