摘 要:目的探讨环加氧酶-2(COX-2)在猪腹部肠管火器伤后心肌组织中表达的变化及其意义。方法健康长白仔猪42头,随机等分为对照组和腹部肠管火器伤后1、2、4、8、12和24h实验组,每组6头。用免疫组化图像分析法测定各组心肌组织COX-2的表达,同时测定血清LDH、CK—MB水平,在光、电镜下观察各组心肌组织学变化。结果伤后各组心肌中COX-2的表达明显高于对照组。伤后各组血清LDH、CK—MB水平均高于对照组。伤后8、12、24h组光镜下出现逐渐加重的心肌细胞水肿、变性;电镜下4、8、12、24h组出现逐渐的线粒体肿胀、溶解;对照组光、电镜下未见明显的损伤性变化。结论腹部肠管火器伤后心肌组织中COX-2表达增强,与心脏结构和功能损伤基本一致;COX-2的活化在腹部火器伤肠管穿透后继发性心脏损伤中扮演着重要角色。Objective To investigate the expressions of COX -2 and its significance in cardiac tissue after intestinal firearm wound. Methods A total of 42 Chang - Bai piglets were randomized into 7 groups: control group and wounded 1 hour group, 2 hour group, 4 hour group, 8 hour group, 12 hour group, 24 hour group. The model of intestinal firearm wound were established in wounded groups. The expressions of COX -2 in cardiac tissue were measured with immunohistochemical staining and image analysis in all groups. Levels of serum LDH and CK - MB were determined. The histological change of cardiac tissue were observed with light microscope and electron microscope(EMS). Results Levels of cardiac COX - 2 expression in wounded groups were significantly elevated compared to control group (P 〈 0.05 ), so were the levels of serum LDH and CK- MB. The experimental group of 8 h, 12 h, 24 h appeared gradually aggravated cardiac cells edema and degeneration under light microscope, mitochondrial appeared gradually aggravated swelling and dissolved under EMS in 4 h, 8 h, 12 h, 24 h group. There were no obviously change in the injury in the control group under light microscope and EMS. Conclusion After intestinal perforations due to abdominal firearm wound, The expressions of COX- 2 in cardiac tissue increases and it is positive correlation with cardiac morphological and functional injury; COX -2 can play an important role in subsequent heart injury after the wound.
