摘　　要：目的:了解目前四川省医疗机构部分药品采购困难的真实原因,为制定和完善四川省现行的药品采购制度和其他相关医改措施及有关决策提供参考。方法:对全省391家医疗机构进行问卷调查,收集各医疗机构采购困难药品的通用名称、剂型、规格、生产企业、实际采购价(或历史采购价)、年采购量、采购困难的原因等信息,并进行分析。结果:不限价药品数量占采购困难药品前50位中的38%,限价药品占52%,自主采购药品占10%;从药品价格水平看,低价药品(≤10元)占采购困难药品前50位中的76%,非低价药品(>10元)占24%。结论:全省各级医疗机构在现行的采购制度下确实存在部分药品采购困难的现象,采购困难药品主要为限价药和低价药,各有关部门应通过推行相应调价机制、改革生产配送模式等措施加以应对,以保证临床药物治疗的顺利实施。OBJECTIVE： To probe into the reasons for the purchase difficulty of some drugs in Sichuan medical institutions for references of establishing and improving the current drug purchase system and other medical reform-related measures as well as the related decision making. METHODS： A questionnaire survey was performed among 391 medical institutions in Sichuan to collect the data of the drugs with purchase difficulty including generic names, dosage, specification, manufacturing enterprises, actual purchase price （or historical purchase price）, annual purchase amount, the reasons for purchase difficulty, the data collected were subjected to analysis. RESULTS： Among the top 50 drugs with purchase difficulty, 38% were unlimited in drug price, 52% were limited in drug price, and 10% were independently purchased drugs; in terms of the drug price level, row-cost drugs （≤10 Yuan） represented 76% of the top 50 drugs with purchase difficulty and non-low-price drugs （〉10 Yuan） represented 24%. CONCLUSION： Under the current purchase system, there are purchase difficulty for some drugs in the medical institutions at all levels in Sichuan province; The drugs which had purchase difficulty were mainly price-limited drugs and low-price drugs. Therefore, it is urgent for the concerned departments to take effective measures such as providing more supply for the raw material of blood products, setting and enforcing the corresponding price-adjusting mechanism and reforming production-delivery mode to ensure the smooth practice of clinical drug treatment.
