卵巢动脉血流与卵母细胞回收数量和质量的关系

摘　　要：目的:探讨体外受精-胚胎移植(IVF/ICSI-ET)控制性超排卵中,卵巢动脉血流与回收卵母细胞数量及其质量的关系。方法:应用前瞻性研究法,随机选择164个超排卵周期,经阴道彩色多普勒B型超声,监测月经第2天(M2)和HCG日卵巢动脉血流S,D,PI、RI和S/D。分析回收卵母细胞数和成熟卵母细胞数与卵巢动脉血流之间的关系。结果:164个超排卵(COH)周期共回收卵母细胞1 714枚,其中成熟卵母细胞1 334枚。根据回收卵母细胞数量和成熟卵母细胞数量分别分成4个组,M2和HCG日,不同回收卵母细胞数4个组的PI、RI和S/D值的波动范围是一致;同样不同回收成熟卵母细胞数4个组的PI、RI和S/D值的波动范围也是一致。但是随着回收卵母细胞数或成熟卵母细胞数的增加,M2和HCG日卵巢动脉血流PI、RI和S/D逐渐降低。结果显示,M2日,当PI<0.62、RI<0.48者,超排卵后可能回收到更多的卵母细胞;而PI<0.60、RI<0.46者,超排卵后可能获得更多的成熟卵母细胞。结论:在COH周期中,卵巢动脉血流与回收卵母细胞数和成熟数密切相关;M2和HCG日,卵巢动脉血流PI、RI值可作为评估超排卵结果的有效指标。Objective ：To explore relationship between the ovarian artery blood flow and the number and quality of oocytes retrieved in controlled ovarian hyperstimulation （COH） cycles. Methods：By prospective study, the ovarian artery blood flow ,which included S, D, PI, RI, and S/D,was detected by transvaginal color Doppler in 164 COH cycles both on second day of menstruous cycle（M2） and the day of HCG administration. And he relationship between ovarian artery blood flow and the number and quality of oocytes retrieved. Results：In 164 COH cycles, 1714 oocytes were retrieved, in which 1334 oocytes is maturation oocytes. Based on the number of oocytes retrieved, results were divided into four groups, and it did Based on the number of maturation oocytes. On M2 day and the day of HCG administration, range of PI,RI and S/D were agreement in four groups involved oocytes retrieved and the same result was obtained in other four groups involved maturation oocytes. But PI,RI and S/D decreased gradually as the number of oocytes retrieved and maturation oocytes increased. Result indicated that much oocytes would be retrieved in COH cycle on M2 day if there exist PI〈0.62 and RI〈0. 48, and much maturion atoocytes would also be get on M2 day if there exist PI〈0.60 and RI〈0.46. Conclusion： In COH cycles, ovarian stromal artery blood flow is closely related to oocytes retrieved and maturation oocytes,PI,RI are available guidelines to evaluate result of COH.
