血运重建术对急性心肌梗死患者心脏胶原重塑的影响

摘　　要：目的评价血运重建干预对急性心肌梗死(AMI)后心脏胶原重塑的影响。方法选择在我院2005-2007年住院的AMI患者65例,依据不同干预方法分为常规治疗组(20例)和血运重建组(45例),48例健康体检者为正常对照组。采用酶联免疫法测定对照组和所有AMI患者在发病后1周、3个月及6个月时点的血清Ⅰ型前胶原羧基端肽(PⅠCP)、Ⅲ型前胶原(PCⅢ),并进行组间和组内的比较。结果⑴与正常对照组比较,常规治疗组和血运重建组(1周和3个月时点的PⅠCP除外)PⅠCP和PCⅢ均有显著升高。⑵与常规治疗组比较,血运重建组各时点的PⅠCP和1周时点的PCⅢ显著降低。⑶常规治疗组各时点和血运重建组各时点的PⅠCP/PCⅢ比值均低于对照组,血运重建组6个月时点PⅠCP/PCⅢ比值高于本组1周和3个月时点。结论AMI后胶原代谢亢进,参与了心脏胶原重塑,血运重建干预可降低AMI后的血清PⅠCP和PCⅢ含量,对胶原重塑有一定的抑制作用。Objective To evaluate the effect of revascularization on heart collagen remodeled patients with acute myocardial infarction （AMI）. Methods According to the different therapies, 65 patients with AMI were randomly divided into 2 groups which were conventional therapy group （n= 20） and revascularization group （n= 45）. 48 healthy persons constituted the normal control group. The serum of concentration of PⅠCP and PCⅢ were measured by Elisa on the 1 week, the 3 months and the 6 months after AMI. Results In comparison with normal group, PⅠCP and PCⅢ were significantly in creased after AMI （except PⅠCP on 1 week and 3 month of the revascularization group）. Compared with the conventional therapy group, PⅠCP of revascularization group and PCⅢ of 1 week of it were significantly decreased. After AMI, PⅠCP/PCⅢ was decreased, compared with the normal control group. In revaseularization group, PⅠCP/PCⅢ on 6th month increased than that on 1 week and 3 months. Conclusion The collagen hypermetabolism after AMI participates in the pathologic process of heart collagen remodeled. The revaseularization may reduce the serum concentration of PⅠCP and PCⅢ after AMI, which may have a certain inhibiting effect on collagen remodeled.
