应用锁定钢板治疗52例胫骨远端粉碎性骨折的临床研究

摘　　要：目的分析锁定钢板在治疗胫骨远端粉碎性骨折中的疗效。方法采用锁定钢板内固定术治疗52例胫骨远端粉碎性骨折,术后对其疗效进行评定。结果术后平均随访18个月,临床疗效分析按照Johner-wruch标准:其中优34例,良10例,可6例,差2例。50例骨折骨性愈合,无畸形愈合,2例延迟愈合,2例皮肤坏死并感染,无小腿筋膜间隔综合征发生,内固定物无松动或断裂。结论应用锁定钢板治疗胫骨远端粉碎性骨折具有创伤小,操作简便,内固定坚强,手术时间短等优点,能早期进行功能锻炼,是临床治疗胫骨远端粉碎性骨折的理想方法。Objective To analyze the effect of the locking steel plate on the comminuted fracture in distal tibia. Methods Using the locking steel plate fixation for treatment of 52 cases of distal tibial Comminuted fracture,postoperative evaluation of its efficacy then can be performed. Results The mean follow-up period is 18 months, the result of clinical efficacy analysis according to Johner- wruch criteria have be seen：excellent in 34 cases,good in 10 cases,passable in 6 cases and poor in 2 cases. 50 cases of bone fracture have been healed, no deformed healing have been found. There are two cases of delayed healing and two cases of skin necrosis following infection. There are no leg compartment syndrome and internal fixation loosening or fracture. Conclusion The use of the locking steel plate fixation on distal tibial comminuted fracture has less trauma and simple operation. It has also strong fixation and shorter operative time, etc. Following early functional exercise it is the ideal clinical treatment of distal tibial comminuted fracture.
