摘　　要：目的了解医院抗菌药物的使用情况,促进合理用药。方法采用回顾性调查方法,分别随机抽取某三甲医院门诊普通处方200份和出院患者病历120份。对门诊患者合理用药国际指标的处方指标、住院患者抗菌药物不合理用药表现进行调查分析,同时对全年抗菌药物用药频度(DDDs)及金额进行排序。结果抗菌药物DDDs前3位为头孢唑林、甲硝唑、头孢美唑钠;头孢菌素类应用最多,占抗菌药物种类的53.85%,金额排序前3位为拉氧头孢、头孢曲松、头孢唑肟钠。门诊患者抗菌药物使用基本合理;住院患者抗菌药物使用不合理主要表现为选择不合理(40.00%)、给药次数不合理(46.67%),尤其是手术给药时机不当的问题较突出。结论该院抗菌药物使用基本合理,还应进一步加强其使用管理和监督,促进临床医生合理用药。Objeetive To investigate the use of antibiotics and to evaluate the rationality of drug use in one 3A- grade hospital. Methods A retrospective study was conducted, in which 200 adult outpatients＇ common prescriptions and 120 case histories of discharged patients in this hospital were randomly selected. The investigation of antibacterials for the prescription indicators of the outpatient international prescription indicators for rational drug use and the manifestations of inpatients＇ non- rational use of drugs, DDDs and the consumption sum sequence of antibacterials used in the whole year were conducted. Results In this hospital the top three antimicrobial drugs of DDDs was cefazolin, metronidazole, cefmetazole sodium, and the consumption sum of the top three was latamoxef, ceftriaxone, ceftizoxime sodium. Outpatients＇ drug use in this hospital was basically reasonable. The manifestations of inpatients＇ irrational use of antibiotics were inappropriate selection （40.00%） and unreasonable administration times（46. 67% ）. The problem of medication time in surgery was most prominent. Conclusion In this hospital the antibiotic drug use was overall basically reasonable. However, management and supervision of the antibacterials use should be strengthened to promote rational drug use.
