儿童肥胖症临床特点与代谢综合征分析

摘　　要：目的观察儿童单纯性肥胖的临床特点,探讨其与代谢综合征(MS)的相关性。方法选取体重指数(BMI)≥28kg/m2的单纯性肥胖儿童94例,匹配同班同龄同性别非肥胖儿童为研究对象,调查其临床表现、家族史、生活习惯、运动量,测定体血压、血脂、血糖等,考查其与改良儿童MS相关指标的吻合情况。结果(1)肥胖儿童中36.17%表现为头晕、乏力、嗜睡等症状,84.02%喜食油炸甜食,79.79%多静少动,24.49%有肥胖家族史;(2)肥胖儿童收缩压和脉压差高于非肥胖儿童(P<0.05);(3)肥胖儿童甘油三酯(TG)、胆固醇(TC)、空腹血糖(FPG)及血压高于非肥胖儿童(P<0.05),肥胖儿童与MS指标的吻合度高于非肥胖儿童(χ2=48.080,P=0.000)。结论儿童肥胖是遗传因素与环境因素交互作用的结果。肥胖儿童血脂、血糖、血压异常,与MS联系密切。合理饮食、加强锻炼是预防肥胖、截断MS发生的关键。Objective To observe the clinical features of children obesity,and to study the correlation with metabolism syndrome. Methods The objects were the 94 children with obesity whose body mass index（BMI）≥28kg/m2and normal children matched in class,age and sex. Clinical symptoms, family history,living habits,physical activities were investigated. Blood pressure, serum lipid, blood sugar were examined. Results （1） All over the children with obesity： 36.17% children had dizziness, fatigue, sleepiness; 79.79% children were rather resting than exercise;24.49% children had positive family history of obesity. （2） Systolic pressure and pulse pressure of children with obesity were higher than normal children. （3）Triglyceride, cholesterol, fasting plasma glucose of children with obesity were higher than normal children（P 〈0.05）. Features of children with obesity matched well with metabolism syndrome indexes（x2= 48. 080, P =0. 000）. Conclusion Obesity is the result of interaction of heredity and environmental factors. Children with obesity are abnormal on blood pressure, serum lipid, blood sugar, and closely related to metabolism syndrome. Eating a balanced diet, more exercise is a reasonable way to prevent obesity and metabolism syndrome.
