摘  要:目的探讨胆道相对热缺血-再灌注损伤对大鼠肝移植早期胆汁成份的影响。方法将24只SD大鼠随机分为假手术组(A组)、胆道相对热缺血0min组(B组)和60min组(C组)。建立大鼠自体原位肝移植胆道外引流模型,检测各组术后第0、1、3、5、7天胆汁中总胆汁酸(TBA)浓度、磷脂(PL)浓度、TBA/PL值,及各亲水性、疏水性胆盐的摩尔分数。结果在术后早期同一时点,A、B两组的TBA浓度、PL浓度、TBA/PL值、各亲水性及疏水性胆盐的摩尔分数均无统计学差异(P>0.05)。与A、B组相比,C组术后早期胆汁中TBA浓度及PL浓度下降(P<0.05),TBA/PL值升高(P<0.05),但均在7d内恢复正常;C组术后早期疏水性胆盐TC、TDC、TCDC的摩尔浓度升高(P<0.05),亲水性胆盐T-β-MC的摩尔分数下降(P<0.05),均在3d内恢复正常。结论胆道相对热缺血-再灌注损伤使肝移植术后早期胆汁中胆盐比例增高,特别是疏水性胆盐比例增高,胆汁毒性增强;术后第7天时胆汁成分即可恢复正常,胆汁毒性消失。Objective To study the effect of relative warm ischemia-reperfusion injury(RWIRI)on early-stage bile secretion after liver transplantation in rats.Methods Twenty-four rats were randomly divided into three groups of sham-operation(group A),relative warm ischemia for 0 min(group B)and 60 min(group C).An orthotopic autologous liver transplantation model was established.The concentration of total bile acid(TBA)and phospholipid(PL),the TBA/PL ratio,and the mole fractions of hydrophilic and hydrophobic bile acids were examined on the days of 0,1,3,5,and 7.Results No significant differencs of TBA and PL concertation,TBA/PL ratio,and the mole fraction of hydrophobic and hydrophilic bile salts were found between group A and B.Compared with groups of A and B,group C had a significant decrease in TBA and PL concentration(P〈0.05),and a significant increase in TBA/PL ratio(P〈0.05)in the early stage,which recovered in 7 days after transplantation.After transplantation,the mole fraction of hydrophobic bile salts of TC,TDC and TCDC increased,while T-β-MC decreased,but recovered within 3 days.Conclusion An increased proportion of bile salts(especially hydrophobic bile salts)caused by the RWIRI can enhance the toxicity of bile.The bile secretion function is able to recover after seven days after liver transplantation,and the bile juice is no longer toxic.
