摘　　要：目的探讨3.0T MR扩散加权成像及其表观扩散系数(ADC值)对胰腺癌的诊断价值。方法对经手术病理证实的胰腺癌患者30例及对照组30例行DWI检查(b值=800s/mm2),测量肿瘤组织、瘤周组织及对照组正常胰腺组织的ADC值,并对所得结果进行统计学检验。结果①肿瘤组织、瘤周组织和正常胰腺组织的平均ADC值分别为(1.494±0.273)×10-3mm2/s、(1.631±0.281)×10-3mm2/s和(1.778±0.237)×10-3mm2/s。单因素方差分析显示肿瘤组织与正常胰腺组织间差异有统计学意义,而肿瘤组织与瘤周组织间平均ADC值差异无统计学意义。②以肿瘤组织平均ADC值95%可信区间的上限(1.622×10-3mm2/s)作为诊断胰腺癌的上限阈值点,诊断的灵敏度、特异度、准确率、阳性预测值、阴性预测值分别为73.33%、86.67%、80.00%、84.62%、76.47%,ROC曲线下面积Az=0.800。结论胰腺癌组织与正常胰腺组织间平均ADC值差异具有统计学意义,测量ADC值对于诊断胰腺癌具有一定帮助。Objective To evaluate the ability of diffusion-weighted imaging（DWI）and the apparent diffusion coefficient（ADC）value in diagnosis of pancreatic cancer at 3.0T MR.Methods The diffusion-weigted sequences with b=800 s/mm ^2 were performed in 30 patients of pancreatic cancer confirmed pathology,and in 30 control subjects with normal pancreas as well.ADC value of the cancer,pericancerous tissue and normal pancreas were recorded and statistically analyzed with ANOVA and ROC analysis.Results The mean ADC value of cancer,pericancerous tissue and normal pancreas was（1.494±0.273）×10^-3 mm ^2/s,（1.631±0.281）×10^-3 mm^ 2/s and（1.778±0.237）×10^-3 mm ^2/s,respectively.ADC value of pancreatic cancer was significant different from that of the normal pancreas,while there was no significant difference between pancreatic cancer and pericancerous tissue.The one-side upper limit of 95% confidence interval of mean ADCs（1.622×10^-3 mm^ 2/s）was adopted as the point to diagnosis of pancreatitis.The area under ROC curve was Az=0.800.Conclusion ADC values are helpful to differential diagnosis of pancreatic cancer and normal pancreas.DWI may be do some contribution to the diagnosis of pancreatic cancer.
