摘 要:目的:研究停吸七氟醚后环路内给不同氧流量对其肺泡排出过程的影响及其与苏醒的关系。方法:28例择期手术病人随机分为:Ⅰ组(n=8),停吸七氟醚后给O22L/min;Ⅱ组(n=10)、Ⅲ组(n=10)则分别给O24L/min和6L/min。结果:三组七氟醚呼末浓度/停吸前呼末浓度(FA/FAO)随时间的曲线经拟合符合对数方程式LgFA/FAO=A±Bt,经计算七氟醚呼末浓度降低50%(FA/FAO=0.5)、降低80%(FA/FAO=0.2)和降低90%(FA/FAO=0.1)的时间以Ⅲ组最快,Ⅱ组次之,Ⅰ组最慢(组间P<0.01)。在停吸20min内患者出现肢动、睁眼的时间快慢以Ⅲ组>Ⅱ组>Ⅰ组,但肢动、睁眼时呼末七氟醚浓度三组间无显著差异(P>0.05)。结论:停吸七氟醚后随氧流量增加,七氟醚排出显著加快,苏醒时间亦明显缩短。Objective: The pulmonary expelling dynamics of sevoflurane and its relation to awakening response at different O 2 flows were investigated after anesthesia. Methods: 28 adult surgical patients were assigned randomly to three groups: group Ⅰ ( n= 8), O 2=2L/min, group Ⅱ ( n= 10), O 2=4L/min and group Ⅲ ( n= 10), O 2=6L/min. Results: The curve of end tidal concentration (F A/F AO ) time of sevoflurane could be best fitted to a logarithmic equation: Lg(F A/F AO )=A±Bt, 0<t≤20min. The time course when the F A of sevoflurane decreased by 50% (F A/F AO =0.5), by 80% (F A/F AO =0.2) and by 90% (F A/F AO =0.1) was as follows: groupⅢ<groupⅡ<groupⅠ ( P< 0.01). Although the time when patients moved and eye opened during awakening process was shorter in groupⅢ than that in groupⅡ and groupⅠ, the related sevoflurane concentration at the appearence of movement and eye open failed to show any significant difference between groups ( P> 0.05). Conclusion: The process of sevoflurane expelled from the lungs can be speeded up significantly as O 2 flow increased after anesthesia, meanwhile the awakening time can be shortened as well.
