应用B-Twin融合器治疗腰椎不稳症的疗效观察

摘　　要：目的应用B-Twin融合器治疗腰椎不稳症,评价其疗效和价值。方法对34例腰椎退变性失稳症患者,采用B-Twin融合器进行后路椎间融合手术,共融合34例36个椎间隙。对所有病例术前、术后复诊时进行JOA评分、X线片椎间隙高度测量,并对融合情况进行考察。结果术后随访时间6～18个月、平均9个月,34例患者术前JOA评分平均(7±1.36)分,术后复诊时平均(22±3.5)分,较术前有显著改善。结果显效26例,有效8例,显效率76.5%。X线片检查显示,平均椎间高度术前(7.5±2.0)mm,术后(11.5±1.3)mm,较术前有显著性差异(P<0.05);术后6～12个月椎间高度(11.0±1.5)mm,较术前仍有显著性差异(P<0.05),动力位X线片显示腰椎融合率88%,B-Twin下沉率23.5%,未发现B-Twin移位。结论应用B-Twin融合器治疗腰椎退变性不稳症,是一种有效方法,严格把握适应证是确保手术长期疗效的前提,手术技巧、B-Twin的最佳大小、植骨技术和数量是确保其近期手术效果的关键。Objective To treat lumbar spinal instability syndrome using B-Twin intervertebral cage and evaluate its effects.Methods From March 2006 to March 2008,34 patients with lumbar spinal instability syndrome underwent PLIF with B-Twin intervertebral cage. Thirty-six intervertebral spaces were treated with this method. Results All patients were followed up from 6 to 12 months （averagely 9 months）. In 34 patients, the score of JOA was （7±1.36） before operation and （22±3.5） at follow-up date after operation averagely. The ex- cellence rate of JOA scores was 76.5%. Clinical results were satisfactory according to JOA scoring system, and so were X-ray data. The average intervertebral height changed from （7.5±2.0） mm preoperatively to （11.5±1.3） mm postoperatively, while （11.0±1.5） mm when followed up in 6-12 months under radiological detection. The difference was significant at the time measured preoperatively and postoperatively or at follow-up date （P〈0.05）. The fusion rate was 88% observed on dynamic X-ray photo. Prosthesis sink rate of B-Twin was 23.5%. No shifting was detected.Conclusion B-Twin intervertebral cage is an effective method on treating lumbar spinal instability syn- drome. The premise to ensure long-term curative effect is strict operation indication, while the key to ensure current curative effect is ex- cellent operative technique, appropriate size of B-Twin intervertebral cage and bone grafting skill.
