标准化尘螨变应原疫苗舌下免疫治疗变应性鼻炎的临床研究

摘　　要：目的评价标准化粉尘螨变应原疫苗滴剂舌下含服治疗变应性鼻炎1年的疗效及安全性。方法采用前后对照,比较97例粉尘螨变应性鼻炎患者在接受1年免疫治疗前、后的症状评分、药物评分、鼻炎相关生活质量评分,评价舌下含服免疫治疗的疗效;并计算不良反应发生率,评价其安全性。结果治疗1年后,97例患者症状评分和药物评分较治疗前均明显减少(P<0.01),有效率79.4%;鼻炎相关生活质量较治疗前明显改善(P<0.01);其中5例(5.15%)出现局部不良反应,5例(5.15%)轻微全身不良反应,未出现严重全身不良反应。结论标准化变应原免疫滴剂舌下含服是治疗尘螨引起的变应性鼻炎的一种有效治疗方法,并具有较好的安全性。OBJECTIVE To evaluate the efficacy and safety of sublingual immunotherapy with Dermatophagoides farinae standardized extract for the treatment of allergic rhinitis for 1 year. METHODS 97 patients with allergic rhinitis caused by Dermatophagoides farinae were enrolled in this self-contrast study. Clinical efficacy was evaluated by symptom scores,pharmacological score and life quality scores after one year of sublingual immunotherapy and compared with pre-treatment scores. RESULTS A clinically significant reduction in symptom and medication scores in these 97 patients compared with those of pre-treatment was found（P0.01）. All items of quality of life were improved significantly（P0.01）. 5 local adverse reactions including oral/sublingual swelling, sublingual tinglingnumbness and oral itching and 5 slight systemic adverse reactions occurred in total subjects,while no fatal systemic reaction occurred. CONCLUSION Sublingual immunotherapy with Dermatophagoides farinae standardized extract is an effective treatment for patients suffering from allergic rhinitis; individualization of dosage is recommended to improve tolerance and security.
