颏下颈阔肌成形术治疗重颏与颈部松垂畸形

摘　　要：目的探讨颏下颈阔肌成形术治疗重颏与颈部松垂畸形的临床效果。方法采用颏颈角切口，先行颈前吸脂，然后在直视下分离颈前皮瓣，去除颈阔肌内侧缘之间及其深面的过多脂肪，显露颈阔肌内侧缘后用3-0涤纶线间断缝合颈阔肌两内侧缘。结果自2007年1月至2009年4月，对23例重颏及颈部松垂的求美者实施手术。术后3～4d术区肿胀最明显，4～5d开始消肿，7～8d基本消退，无血肿形成。随访其中的12例求美者3～9个月，切口隐蔽，无瘢痕增生，效果满意。结论颏下颈阔肌成形术操作安全，对重颏与颈部松垂畸形有较好的疗效。Objective To explore the clinical effect on submentum platysmaplasty for the treatment of double-chin and cervical ptosis deformity. Methods Through the mental cervical angle incision, cervical liposuction was performed firstly, the flap of anterior neck was separated under euthyphoria and then the excessive fat around platysma medial borders and subplatysma was excised and the exposed medial borders of platysma was sutured finally by interrupted suture with 3-0 dacron. Results From January 2007 to April 2009, 23 cases were treated in this study. The most obvious swelling appeared at 3 - 4 days postoperatively, detumescence began at 4 - 5 days and essentially disappeared at 7 - 8 days. There was no hematoma in any cases. Follow-up was performed in 12 cases for 3 -9 months, all were satisfied with the concealed incision without scar hyperplasia. Conclusion Submentum platysmaplasty is safe and effective, which can improve double-chin and cervical ptosis deformity.
