去黏膜细胞同种异体气管在犬气管重建中应用的研究

摘  要:目的探寻去黏膜细胞异体气管在犬气管重建中的应用价值。方法应用改良Courtman消化法去除犬异体气管黏膜细胞成分,观察去细胞效果;将去黏膜细胞气管原位移植至宿主犬进行气管缺损重建,观察宿主犬存活情况、气管缩窄程度及组织学变化。以新鲜未处理气管移植作为对照组。结果实验组宿主犬均存活至90d处死,未出现明显咳嗽及窒息症状,而对照组宿主犬均于33d内(平均26.2±5.1d)因窒息死亡。实验组气管缩窄率(0.27±0.07)X100%明显小于对照组(0.62±0.05)X100%。实验组移植段气管组织学观察管腔内肉芽组织较少,无明显软骨破坏,周围少量淋巴细胞浸润,管腔内壁可见移行的上皮细胞生长。而对照组软骨破坏严重,肉芽组织增生及淋巴细胞浸润情况严重。结论去黏膜细胞异体气管原位移植修复犬气管缺损,可有效降低宿主犬对移植段气管的免疫排斥反应程度,并获得较长的生存时间及较高的生存质量。Objective To explore the applied value of allogenic trachea without epithelium and mixed glands in reconstruction of dog's trachea. Methods All cells in the mucous membrane of allogenic trachea were removed by modified Courtman method, then the treated trachea was transferred by the method of orthotopic transplantation into the defected dog's trachea, the fresh trachea which did not remove the cells in the mucous membrane was as the control group. Dogs' survival, tracheal eoaretation degree and its histological changes were observed. Results The dogs in the experimental group were sacrificed at 90 days without such severe symptoms as cough and asphyxia, while the dogs in the control group all died of asphyxia within 33 days ( 26.2 ± 5.1 d). The stenosis ratio (0.62 ± 0.05 ) x 100% in the control group was higher than that in the experimental group (0.27 ± 0.07 ) x 100%. Compared with the control group, histologyieal examinations in the experimental group showed fewer granulation tissues and lymphocyte infiltration and no severer necrosis of cartilage ring in the transplanted trachea. Conclusion The removal of cells of epithelium and mixed glands in allogenic trachea rings can reduce the immune reaction and improve the quality and survival time of host dogs under orthotopic transplantation.
