不明原因发热患者血清蛋白变化及其蛋白电泳分析

摘　　要：目的探讨血清前清蛋白(PA)、清蛋白(ALB)及其蛋白电泳检测在不明原因发热患者病因判断和治疗中的应用价值。方法收集26例健康体检者和86例不明原因发热患者血清,测定其血清前清蛋白、清蛋白并进行蛋白电泳分析。结果不明原因发热患者组血清PA、ALB降低,与对照组相比差异均有显著性(P<0.01),肿瘤性疾病与感染性疾病和血管结缔组织病相比PA降低较显著(P<0.05)。蛋白电泳图谱分析显示患者组ALB与正常组比较显著性降低(P<0.01);感染性疾病多表现为ALB降低、α2、β球蛋白升高(P<0.01);肿瘤性疾病多表现为ALB降低、α1球蛋白升高(P<0.01)和γ球蛋白升高(P<0.05);血管结缔组织病多表现为ALB降低、α2球蛋白升高(P<0.01)和α1、β球蛋白升高(P<0.05)。肿瘤性疾病与感染性疾病和血管结缔组织病相比α2球蛋白显著降低(P<0.05)。结论通过测定不明原因发热患者的蛋白变化,可对患者有针对性地进行进一步的检查,从而尽快地确定不明原因发热的病因,给临床治疗提供一定的帮助。Objective To study the diagnostic value of detecting pre - albumin, albumin and protein electrophoresis in the serum of patients with fever of unknown origin. Methods Serum pre - albumin, albumin and protein eleetrophoresis were detected in 86 patients with fever of unknown origin and 26 controls. Results The level of albumin and pre - albumin in fever of unknown origin group was lower than that of control group （ P 〈 0.01 ）. Comparing with the infectious and the vascular connective tissue diseases groups, the level of pre - albumin in patients with neoplastic diseases was lower （ P 〈 0.05 ）. The chart of protein electrophoresis showed that the levels of albumin significantly decreased in the patient group （ P 〈 0.01 ）. Comparing with the control group, the level of albumin in patients with infectious diseases was lower （P 〈 0.01 ） and cq, [3 globulin was higher（ P 〈 0.01 ）. In patients with neoplastic diseases ,the level of albumin often significantly decreased （ P 〈 0.01 ）, α1 globulin significantly increased （ P 〈 0.01 ） and y globulin significantly increased （ P 〈 0. 05 ）. In patients with the vascular connective tissue disease, the level of albumin often significantly decreased （ P 〈 0.01 ） , % globulin significantly increased （P 〈0.01 ） and α1, β globulin significantly increased （P 〈 0.05 ）. Comparing with neoplastic diseases groups, the level of α2 globulin in patients with the infectious and the vascular connective tissue diseases was higher （ P 〈 0.05 ）. Conclusion Determination of pre - albumin, albumin and protein electrophoresis has reference value to find the cause of patients with fever of unknown origin. We can classify the patients primarily, and it can provide definite help for clinic diagnosis.
