摘  要:目的:分析HCG日注射剂量不同和12h后血β-HCG水平的差异,探讨其对早期胚胎发育的影响。方法:回顾分析2008年10月2009年4月721个胚胎移植周期注射HCG后12h的血β-HCG水平。结果:按HCG日注射10000IU、8500IU和6500IU分组,随着HCG注射剂量降低,血β-HCG水平随之降低,差异有统计学意义。获卵数,成熟卵母细胞(MⅡ)率均呈降低趋势,IVF中6500IU组受精率显著低于其它两组(P〈0.05),ICSI中各组没有显著差异;按注射HCG后12h血β-HCG值分组,IVF-ET和ICSI中,患者年龄、不孕年限、使用Gn天数和支数均无显著差异。β-HCG值〈100 mIU/ml组的体重指数最高,〉200 mIU/ml组最低,3组差异有有统计学意义(P〈0.05)。结论:HCG对卵子的最后成熟很关键,血β-HCG水平可能对胚胎质量没有显著影响,仅影响获卵数和MⅡ率。临床上对于体重指数较高的患者可适当增加HCG剂量,强调治疗方案个体化。Objective:To analysis different doses on the day of HCG injection and the differences of blood β-HCG level after 12 hours of the HCG injection and investigate the effects of HCG on the early embryonic development.Methods:The data of 721 cycles of IVF-ET which had blood β-HCG results after 12 hours of the HCG injection from Oct.2008 to Apr.2009 were analysed retrospectively.Results:Grouping analyzing the dose of injection of HCG on 10000IU,8500IU and 6500IU,with lower doses of HCG injection,blood levels of β-HCG also lower,the difference was significant.And the number of oocyte,MⅡ rate,fertilization rate showed a decreasing trend.The group of 6500IU's fertilization rate was significantly lower than that of the other two groups in IVF(P〈0.05),all of these items in the ICSI had no significant difference;to divided 3 groups with blood β-HCG value,there were no significant difference between the groups of the patient's age,years of infertility,the doses and days of use of Gn in the IVF-ET and ICSI.β-HCG values〈100 mIU/ml group had the highest body mass index,〉200 mIU/ml in the lowest group,three groups had significant difference(P〈0.05).Conclusion:HCG is critical for final oocyte maturation,blood β-HCG level may be no significant impact on embryo quality,the impact has been only a fewer oocytes and lower MⅡrate.Patients with higher body mass index may be appropriate to increase the dose of HCG,emphasizing individualized treatment programs.
