摘 要:目的应用基于体素的形态测量学方法(VBM)研究先天性耳聋患者的灰质和白质变化情况。资料与方法对18例先天性耳聋患者和22名健康自愿者全脑进行三维磁共振成像数据采集,应用(diffeomorphic anatom-ical registration through exponential lie algebra,DARTEL)对灰质和白质进行VBM分析。以簇体积>100个体素,且P<0.001为差异有统计学意义。结果与健康自愿者相比,耳聋患者灰质体积增大的脑区为左侧颞横回和双侧小脑(P<0.001);灰质体积减小的脑区为双侧楔回、左侧梭状回和右侧中央前回(P<0.001);白质体积减小的脑区为双侧前扣带回、左侧胼胝体、左侧舌回和右侧额中回(P<0.001)。结论先天性耳聋患者灰质和白质发生改变,反映了脑可塑性重组及患者使用手语交流导致脑结构发生的改变。Objective To evaluate changes of brain volume in congenital deaf patients with a voxel-based morphometric analysis. Materials and Methods A 3D fast spoiled gradient echo sequence to collect 196 contiguous 1-mm thick,T1-weighted images in the sagittal plane was performed in 18 congenital deaf patients and 22 volunteers. DARTEL(Diffeomorphic Anatomical Registration Through Exponential Lie Algebra) was applied to complete voxel-based morphometric analysis of grey and white matter. Threshold P〈0.001 with extent threshold 100 voxels was defined as statistical significance. Results Compared to volunteers,increased grey matter volume of brain regions in congenitally deaf patients included left transverse temporal gyrus and bilateral cerebella ( P〈0.001),decreased grey matter volume of brain regions included bilateral cuneus gyri,left fusiform gyrus,and right precentral gyrus ( P〈0.001),and decreased white matter volume of brain regions included bilateral anterior cingulated gyri,left corpus callosum,left lingual gyrus,and right middle frontal gyrus ( P〈0.001). Conclusion Volume changes of grey matter and white matter of brain regions were demonstrated in congenitally deaf patients,which reflected changes of brain structures resulted from cross-modal plasticity of brain.
