摘 要:目的:探讨小凹蛋白-1及白三烯B4在急性冠脉综合症(ACS)患者中表达的变化及其意义。方法:选择45例经冠状动脉造影及临床证实为ACS的患者与20例无冠状动脉病变的对照组进行研究,以流式细胞技术检测外周血单核细胞小凹蛋白-1(Cav-1)的表达水平;采用ELISA检测30例ACS患者及对13例照组血清中白三烯B4(LTB4)的表达水平。结果:ACS组单核细胞Cav-1表达水平显著低于对照组(P<0.01);ACS组血清LTB4水平显著高于对照组(P<0.05)。Cav-1在急性心肌梗死(AMI)与不稳定心绞痛(UAP)组均显著低于非冠心病组(P<0.01);AMI患者血清中LTB4水平显著高于UAP组及非冠心病组(P<0.01)。经多元Logistic回归分析ACS与非冠心病组,显示LTB4总预测准确率70.7%,Cav-1总预测准确率78%,而结合LTB4及Cav-1两类指标总预测准确率上升为80.5%。结论:单核细胞上小凹蛋白-1在ACS患者表达降低,AMI患者血清中LTB4表达上调,结合两类指标对预测发生ACS的准确率比单个指标的评价更有意义。Objective To investigate the expression of Caveolin-1 (Cav-1)and Leukotriene B4 (LTB4) in acute coronary syndrome (ACS). Methods Using flow cytometry, Car-1 expression of peripheral blood mononuclear cells was measured in 45 ACS patients ( diagnosed by coronary angiography and clinical manifestation) and 20 control cases without coronary artery disease. Serum LTB4 was measured by ELISA in 30 ACS patients and 13 control cases. Results Compared with control group, the expression of Car-1 in ACS patients was significandy lower( P 〈 0.01 ) and the level of LTB4 in ACS group was significantly higher( P 〈 0.05 ). Furthermore, the exprssion of Car-1 in patients with acute myocardial infarction(AM1) and unstable angina peetoris(UAP) was extremely lower than that in control group( P 〈 0.01 ), and the level of LTB4 in AMI group was markedly higher than that in UAP and control group( P 〈 0,01). The results of logistic multiple regression on ACS and control group indicated that the overall percentage of LTB4. to predict the two groups was 70.7 %, and overall percentage of Cav- 1 was 78 %, while the overall percentage to predict devated to 80.5 % by the combination of LTB4 and Cav-1. Conclusions The expression of Car-1 on peripheral blood mononuelear cells in ACS patients is down regulated, and the level of LTB4 in AMI is elevated. Contrasted with any single indicator above, combining both indicators to predict the exact rate of occurrence of ACS is more valuable.
