正常女童生殖器超声多参数的检测

摘　　要：目的了解女童从新生儿至青春期内生殖器官发育中超声主要检测参数值的变化规律,建立本地区正常女童生殖器超声多参数正常值。方法应用二维及彩色多普勒超声对571例0～14岁正常女童内生殖器官17项超声参数进行测量:子宫体长(UT-L),厚(UT-TH),宽(UT-W),容积(UT-V),子宫内膜回声;子宫颈长(CX-L),厚(CX-TH),宫体厚度/宫颈厚度比值(FCR),阴道前后壁厚度(V-TH);卵巢长(OV-L),厚(OV-TH),容积(OV-V),卵泡内径(FO-D),卵泡数目(FO-N);子宫动脉收缩期流速(Vmax),子宫动脉舒张期流速(Vmin),子宫动脉阻力指数(RI)。结果青春期前(≤9岁)女童生殖器各项超声参数测值为子宫体长<2cm,厚<0.9cm,宽<1.4cm,子宫体容积<1.5ml,内膜线不显示或欠清晰;宫颈长<1.8cm,宫颈厚<0.8cm,宫体与宫颈厚度比值(FCR)<1.2,阴道壁厚<0.4cm;卵巢长<1.9cm,厚<1.0cm,卵巢容积<1.0ml,卵泡内径<0.4cm,卵泡数目<4个;子宫动脉峰值流速<20cm/s,舒张期流速<2cm/s,子宫动脉阻力指数(RI)>0.9。结论女童生殖器官发育分4个阶段:新生儿期,婴儿早期,7个月至9岁,10～14岁。青春期前(≤9岁)子宫随年龄而缓慢增长,青春期开始后(10～14岁)子宫体增长加速,子宫内膜线呈现,宫体与宫颈厚度比值增加,子宫动脉阻力指数减低。卵巢发育在7岁前处于静止期。Objective To establish the normal range of genital ultrasonographic parameters in healthy girls by exploring the main sonographic parameters and the morphologic development of the internal genitalia from birth to puberty. Methods To measure 17 sonogrphic parameters of the internal genitalia in 571 healthy girls aged from 0 -14 years induding： uterine length, thickness , width and volume, endometrial echo, cervical length and thickness, ratio of fundal and cervical anteroposterior diameter （FCR）, thickness of vaginal wall, ovarian length, thickness and volume, diameter and number of follicle, peak flow velocity, diastolic flow velocity, resistant index（RI） of uterine artery. Results The main sonographic parameters of the girls in prepuberty（ ≤9 years） ： the length of uterine body was 〈2 cm,the thickness of uterine body was 〈0.8 cm,the width of uterine body was 〈 1.4 cm, the volume of uterine body was 〈 1.5 ml, the endometrial echo of uterine was not existed or clear, the length of cervix 〈 1.8 cm, the thickness of cervix 〈 0.8 cm, FCR 〈 1.2, the vaginal thickness 〈 0.4 cm, the length of ovary was 〈 1.9 cm, the thickness of ovary was 〈 1.0 cm, the volume of ovary was 〈 1.0 ml, the diameter of follicle was 〈0.4 cm, the number of follicle was the maximal velocity of uterine artery was 〈20 cm/s, the diastolic flow velocity of uterine artery was 〈 2 cur/s, RI of uterine artery was 〉 0. 9. Conclusions The development of the internal genitalia in girls was classified into four stages： neonatal, early infant, 7 months to 9 years, 10 to 14 years. The uterus growed up slowly with the age in prepuberty（ ≤9 years）. The growth of uterine body was greater than cervix in group of 10-14 years. The endometrial echo was existed, FCR increased and RI of uterine artery decreased. The ovaries were in static status before 7 years.
