摘　　要：目的观察人工诱发性房性早搏与室性早搏对窦性心率震荡的影响。方法50例室上性心动过速患者接受电生理检查时,分别刺激高右房和右室心尖部,刺激周长从600ms直至不应期,步长为50ms。分别计算高右房和右室心尖部的震荡初始(TO)和震荡斜率(TS)值。结果与室性早搏的TO与TS相比,房性早搏的TS值明显减小,TO值明显增大;房性早搏的TS与室性早搏的TS有明显的相关性(r=0.65,p<0.01),而房性早搏的TO与室性早搏的TO之间无明显的相关性。结论与室性早搏相比,房性早搏的心率震荡现象减弱,两者的心率震荡参数TS具有相关性。Objective To determine the effects of manual induced atrial premature beat（APB） and ventricular premature beat（VPB） on heart rate turbulence （HRT）. Methods A total of 50 subjects referred for electrophysiologic evaluation for Paroxysmal Supraventricular Tachycardia（PSVT） underwent a stimulation protocol consisting of single atrial in the high right atrium （HRA） and ventricular extrastimuli in the right ventricular apex （RVAP） with coupling intervals decremented from 600 to the refractoriness of each site at a 50msstep. Turbulence onset（TO） and turbulence slope（TS） after an APB and a VPB were calculated separately. Results After an APB, TS was significantly attenuated （3.23±1.26 vs. 7.69±2.19ms/RR, p〈0.008） and TO was significantly higher （0.97±0.21% vs. -4.76 ±0.57%, p〈0.001） compared with those after a VPB. There was a strong correlation between TS （r = 0.55, p〈0.01） but not between TO （r = 0.16, p〉0.05） after an APB and a VPB. Conclusions HRT was weaker after an APB in contrast to a VPB. There was correlation between TS after an APB and a VBP.
