急性脑梗塞患者血清MMP-9及TIMP-1变化与血淤证的相关性研究

摘　　要：目的探讨急性脑梗塞患者血清MMP-9及TIMP-1变化与血瘀证的相关性。方法对136例脑梗塞患者进行中医证候要素评分及血清MMP-9及TIMP-1测定。结果血清MMP-9比较,血淤证组较非血淤证组显著升高(P<0.01),在脑梗塞急性期的4个时点中,血淤证患者血清MMP-9在前3 d呈升高趋势,在72h以后开始下降;血清TIMP-1比较,血淤证组与非血淤证组均无显著差异(P>0.05)。血瘀证与血清MMP-9显著相关(P<0.01),与血清TIMP-1无明显相关(P>0.05)。结论血清MMP-9与血淤证呈正相关,在脑梗塞急性期前3 d呈升高趋势,72 h后下降,随着血清MMP-9升高,血淤证的程度加重,但血淤证与血清TIMP-1无明显相关。Objective To investigate the relationship between blood stasis syndrome （BSS） defined by traditional Chinese medicine in cerebral infarction and the contents of MMP -9 and TIMP - 1. Methods All subjects comprised 128 patients with acute ischemis stroke, which were based on diagnostic criteria for cerebral Vascular diseases in 2005. Diagnosis for syndromes defined by traditional Chinese medicine were made according to Diagnostic Criteria for Stroke in 1994. ELISA was adopted to detect the contents of MMP - 9 and TIMP - 1 in serum. Results The level of MMP - 9 in serum of patients with CI and BSS was much higher than that in patients without BSS but with CI（P 〈0.01 ）. The content of MMP -9 in BSS patients with BSS and CI was increased in the first three days,but the content of MMP - 9 in these patients was decreased thereafter. The content of TIMP - 1 had no difference between patients with BSS and CI and those patients without BSS but with CI（P 〉 0.05 ）. There was significant correlation between MMP -9 and BSS in patients with cerebral infarction （P 〈 0.01 ）. There was no correlation between TIMP - 1 and BSS in patients with cerebral infarction（P 〉 0.05）. Conclusion There is significant correlation between MMP- 9 and BSS in patients with cerebral infarction. The content of MMP - 9 in BSS patients with BSS and CI present will increase in the first three days, but the content of MMP -9 in these patients will decrease thereafter. The severity of BSS in patients is enhanced with the increase of MMP - 9. There is no correlation between TIMP - 1 and BSS in patients with cerebral infarction.
