摘 要:目的探讨老年2型糖尿病(T2DM)口服降糖药疗效不佳的患者经胰岛素皮下持续输注(CSII)治疗前后动态血糖变化。方法对32例老年T2DM口服降糖药疗效不佳的患者先进行3d血糖监测,再使用胰岛素泵治疗,同时运用雷兰动态血糖监测系统(DGMS)评估治疗前后的血糖情况。结果经1周治疗后,32例患者的空腹、三餐后2h、晚22:00、晨2:00血糖水平明显降低,24h平均血糖值明显下降,24h总体达标时间延长,血糖波动系数下降,与治疗前相比差异有统计学意义(P<0.01)。低血糖发生少,与治疗前相差无统计学意义(P>0.05)。结论DGMS能全面掌握糖尿病患者的血糖波动,与CSII联合应用能更有效地控制血糖,治疗后患者不仅血糖水平明显下降,而且24h总体达标时间延长,血糖波动小,低血糖发生少。Objective To detect the dynamic blood glucose variation of elderly patients with type 2 diabetes mellitus (T2DM) before and after treatment with continuous subcutaneous insulin infusion(CSII) ,who have been treated by oral antihyperglycemic drug(OAD) with unsatisfied therapeutic effects. Methods 32 elderly patients with T2DM who have been treated by OAD with unsatisfied therapeutic effects accept glucose monitoring for 3 days, then they are treated by insulin pump. At the same time,glucose were evaluated by dynamic glucose monitoring system(DGMS). Results After one week,the levels of FPG,2h postprandil blood glucose(2hPG), 10pm, 2am in 32 patients were significantly decreased, the average glucose of 24 hours was lower, the total target time was longer, glucose curve was placid shown (P〈0.01). Hypoglycemic frequency was lower (P〉0. 05 ). Conclusion DGMS can roundly control the fluctuation of glucose, and it can control glucose more efficiently by combining with CSII. After therapy, not only the level of glucose will decrease, but also the total target time was longer, glucose curve was placid shown and hypoglycemic frequency was lower.
