摘  要:目的通过大鼠异位心脏,右肺联合移植建立左室机械辅助减负荷模型,研究并心肌逆向重塑。方法选用Lewis大鼠28只,7只作为正常对照,14只结扎冠状动脉左前降支诱导心衰后完全随机均分为两组:心衰组及心脏移植组。剩余7只大鼠作为受体。心脏移植组将心衰后心脏及右肺移植到受体大鼠的腹部。移植两周后,同时摘取3组大鼠的心脏评价心脏体质量、左室体质量、心肌细胞直径大小,及检测大脑利钠肽(BNP)mRNA的表达。结果心衰后心脏增大,其左室体质量[正常组(0.55±0.02)g,心衰组(0.85±0.02)g,移植组(0.49±0.02)g]和左室心肌细胞直径[正常组(18,5±0.5)μm,心衰组(26.2±1.1)μm,移植组(20.1±0.4)μm]在移植后趋于正常。心衰后左室BNPmRNA表达升高,移植后趋于正常。结论该模型模拟了临床左室辅助减负荷后心肌逆向重塑的效果,且简单及易于成功。Objective To establish a model of left ventricular mechanical unloading by heterotopic heartright lung transplantation in rats, and to explore reverse remodeling effect by left ventricular unloading in heart failure. Methods Twenty-eight Lewis rats were used. Of them, 7 rats were used as normal control group, 14 rats with heart failure were induced 4 weeks after left anterior descending ligation, and were complete randomly divided into two groups: heart failure group and heart transplantation group. The other 7 rats were recipients. The failing heart and right lung were ectopically transplanted into the abdomen of recipients. Two weeks after transplantation, hearts of three groups were harvested and evaluated for heart weight, left ventricle weight, diameter of cardiomyocyte, and expression of brain natriuretic peptide (BNP) mRNA. Results Heart weight, left ventricular weight, myoeyte diameter and level of BNP mRNA increased in heart failure group (0.85±0.02 g, 26.2±1.1 μm), and those parameters decreased to normal group(0.55± 0.02 g, 18.5±0.5 μm) level in transplantation group (0.49±0.02 g, 20.1 ±0.4 μm)(P〉0.05). Conclusion This animal model successfully simulates the unloading effect of left ventrieular assist device, and also appeares easily reproducible.
