超声在小体积乳腺叶状肿瘤和纤维腺瘤鉴别诊断中的价值

摘　　要：目的:探讨超声对小体积(直径<5cm)乳腺叶状肿瘤和纤维腺瘤鉴别诊断的价值。方法:回顾性分析经病理证实、直径均<5cm的12例乳腺叶状肿瘤和96例纤维腺瘤患者的超声表现。结果:囊性变、Ⅱ级和Ⅲ级血流信号(根据Adler半定量法分级)在2组患者中的检出率差异有统计学意义(P<0.05)。其中囊性变和Ⅲ级血流信号诊断的特异性和准确性较高,特异性均为99.0%,准确性分别为91.7%和90.7%。结论:囊性变和Ⅲ级血流信号是鉴别诊断直径<5cm的乳腺叶状肿瘤和纤维腺瘤的重要指标。Ⅱ级血流信号在二者中存在较大重叠,可作为鉴别诊断的参考指标。Objective：To investigate the value of differential diagnosis of small breast phyllode tumor and small breast fibroadenoma by color Doppler ultrasonography. Methods：The characteristics on color Doppler ultrasonography were analysed in 12 cases with breast phyllode tumor and 96 cases with breast fibroadenoma, all masses were less than five centimeters in diameter and confirmed by pathology. Results： The detection rates of cystic degeneration areas, type Ⅱblood flow and type Ⅲ blood flow in two groups were significantly different （P〈0.05）. The specificities of cystic degeneration areas and type Ⅲ blood flow were up to 99.0% and the accuracies were 91.7% and 90.7% respectively. Conclusion：Cystic degeneration areas and type Ⅲ blood flow are two important indicators in the differential diagnosis of small phyllodes tumor and small fibroadenoma. For type Ⅱblood flow, there is a big overlap in the two groups, which can be used as a reference.
