摘 要:目的研究异丙酚对缺血再灌注脑损伤大鼠海马区凋亡因子Bax、Bcl-2及大脑梗死体积的影响,探讨异丙酚对脑缺血再灌注损伤的保护作用。方法Wistar大鼠共40只,随机分为4组,每组10只,S组为假手术组,C组为缺血再灌注损伤对照组,I组为线栓梗塞缺血后异丙酚治疗组,R组为线栓梗塞缺血再灌注后异丙酚治疗组。结果C组、I组、R组的神经功能缺失体征评分无明显差异(P>0.05)。I组和R组的梗死体积比C组明显减小,差异有统计学意义(均P<0.01)。C组海马区单位面积内细胞核Bax阳性表达的细胞数目较S组、I组、R组升高(均P<0.05),S组、I组、R组3组之间比较差异无统计学意义(P>0.05)。I组、R组海马区Bcl-2吸光度测定值分别高于C组和S组(均P<0.05)。但C组与S组之间、I组与R组之间比较差异无统计学意义(均P>0.05)。结论异丙酚可降低缺血再灌注脑损伤大鼠海马区凋亡因子Bax的表达和大脑梗死体积,提高抑制凋亡因子Bcl-2的表达,对缺血再灌注脑损伤有一定保护作用。Objective To observe the effect of propofol on the expression of Bax,Bcl 2 and volume of infarction in rats with ischemie brain damage. Methods Forty Wistar rats were randomly divided into four groups(n= 10 each)as follows:group S, sham operation; group C, ischemia and reperfusion; group I, propofol treatment following the thread occlusion of right common carotid artery and obstruction of the blood flow of ipsilateral middle cerebral artery;group R,reperfusion for 2 h after ischemia by thread occlusion followed by treatment with propofol. Results There was no significant difference in neurologic scores a- mong groups C, I, and R(all P〉0.05). The volume of infarction in group I or R was significantly reduced as compared with that in group C( P〈0.01 ). In group C, the number of cells positive for Bax in rat hippocampi was increased as compared with groups S,I,and R(P〈0.05) ,but there was no significant difference among groups S,I and R(P〉0.05). The A values of Bcl-2 in the rat hippocampi in groups I and R were significantly higher than in groups C and S(P〈0.05), but there was no significant difference between groups C and S, or between groups I and R(P〉0.05). Conclusion Propofol can reduce the expression of Bax in rat hippocampi following ischemic brain damage and the volume of infarction in rat brain. Propofol can also reinforce the expression of Bcl-2,which can inhibit apoptosis,and then generate neuroprotective effects.
