患者术后拔除留置尿管的时机对排尿的影响

摘　　要：目的探讨不同拔除留置尿管时机对泌尿术后患者排尿的影响。方法将180例泌尿术后留置尿管的患者随机分为三组，甲组为膀胱充盈、有强烈尿意时拔除尿管；乙组为在医生下达医嘱后不论膀胱是否充盈即拔除尿管；丙组为排空膀胱后拔除尿管；观察拔除尿管后三组患者的排尿情况。结果拔除尿管后首次排尿时间、膀胱区及尿道疼痛情况3组比较差异均有显著性（P〈0．05），甲组患者白行排尿成功率97％，乙组85％，丙组77％，甲组与乙组或丙组比较差异有极显著性（P〈0．02）。结论膀胱充盈同时有强烈尿意时拔除留置尿管，患者首次排尿出现早，膀胱区、尿道疼痛明显减少，自行排尿成功率高，尿潴留的发生率减少，是留置尿管拔除的最佳时机。Objective To investigate the effects of the different timing of indwelling catheter removal on micturition after urologic surgery. Method Separate randomly 180 cases into three groups, who with Indwelling catheter after the urologic surgery, the first division is removed the indwelling catheter when the bladder is filling with strong micturition desire, the second division is removed the indwelling catheter no matter the bladder filling or not ; the third division is removed the indwelling catheter after emptying the bladder ： then observe the micturition conditions after the removal of the indwelling catheter. Result After the removal of indwelling catheter, there are significant differences （ P〈 0.05 ）, on the first micturition, the conditions of cystic area and urethra., the success rate of micturition by themselves： the fist division is 97%, the second division is 85%, the third division is 77%. the first and second division is much superior than the third division （ F〈 0.02 ）. Conclusion When the bladder is filling with strong micturition desire, it is good timing of removing the catheter, the first micturition is early, less pain on the cystic area and urethra ： the success rate of micturition is higher, uroschesis occurrence is much lower.
