生长抑素及垂体后叶素联合硝酸甘油治疗食管胃底静脉曲张破裂出血疗效分析

摘　　要：目的:评价生长抑素及垂体后叶素联合硝酸甘油治疗肝硬化并食管胃底静脉曲张破裂出血(EGVB)的临床疗效。方法:100例EGVB患者分为三组:垂体后叶素联合硝酸甘油组22例;生长抑素组42例;生长抑素、垂体后叶素、硝酸甘油组(联用组)36例。观察三组的止血成功率、平均止血时间、肝性脑病发生率及不良反应。结果:垂体后叶素联合硝酸甘油组止血率、平均止血时间分别为81.8%(18/22)、(3.33±0.97)天;生长抑素组分别为95.2%(40/42)、(2.23±0.77)天;联用组分别为97.2%(35/36)、(1.8±0.58)天。联用组与生长抑素组比较止血成功率无统计学意义(P>0.05)。三组平均止血时间比较有统计学意义(P均<0.05)。联用组与生长抑素组肝性脑病发生率比较有统计学意义。结论:生长抑素、垂体后叶素与硝酸甘油联合治疗肝硬化EGVB,不但止血效果好,平均止血时间缩短,而且肝性脑病发生率明显低。Objective ：To evaluate the clinical effect of growth inhibin,nitroglycerin plus pituitrin on esophageal and gastric varicose bleeding（EGVB）. Methods： 100 hepatic cirrhosis patients with esophageal and gastric varicose bleeding were divided into 3 groups： nitroglycerin plus pituitrin group （group 1, n=22）, growth inhibin group （group 2, n=42）,growth inhibin, nitroglycerin plus pituitrin group （group 3 ,n=36）.The hemostatic rate ,mean hemostatic time and hepatic encephalopathy rate were observed. Results ：The hemostatic rate,mean hemostatic time were respectively gl.8%（18/22）, （3.33±0.97）d in group I ;95.2%（40/42）, （2.23±0.77）d in group 2;and 97.2%（35/36）, （1.8±0.58）d in group 3. A comparison between the group 2 and 3 had no statistical significance with regard to hemostatic rate （P〉0.05）, and the differences in mean hemostatic time were remarkable among 3 groups （P〉0.05） . Differences in hepatic encephalopathy rate were remarkable between group 2 and 3（P〉0.05）. Conclusion：Growth inhibin, nitroglycerin plus pituitrin group for treating EGVB is effective, and mean hemostatic time is less. Hepatic encephalopathy rate is low.
