摘 要:目的观察老年男性冠心病患者睾酮水平与靶器官损伤的关系及探讨其临床意义。方法选取124例年龄65~80岁的男性患者,分冠心病组62例,对照组62例。检测血清睾酮(testosterone,T)水平,并根据多普勒超声测定颈动脉内膜-中层厚度(IMT),心脏左室重量指数(LVMI),及测量24h尿微量白蛋白(microalbunminuria,MAU).比较两组间T,IMT,LVMI,MAU水平差异及分析T水平与IMT,LVMI,MAU的关系。结果两组间年龄、体重,高血压、高血脂比例,MAU差异无统计学意义(P〉0.05);冠心病组T水平低于对照组(P〈0.01),IMT,LVMI水平分别高于对照组俨〈0.01)。直线相关性分析显示,冠心病组T水平分别与MAU、IMT、LVMI呈负相关(P〈0.01)。对照组T水平分别与MAU、IMT、LVMI呈负相关(P〈0.01).结论老年男性冠心病患者睾酮呈低水平状态,睾酮水平低下可能与靶器官损伤有密切关系。Objective To observe correlations and clinical significance of testosterone (T) with target organ damage in elderly male patients with coronary heart disease (CHD). Methods 124 male patients ranging in age from 65 to 80 years were equally divided into a CHD group and a control group. Serum T level and 24h urinary microalbuminuria (MAU) were measured, and carotid intima media thickness (IMT) and cardiac left ventricular mass index (LVMI) were also measured by Doppler ultrasound. Differences in T, IMT, LVMI and MAU between the two groups were compared, and correlations between T and IMT, LVMI and MAU levels in the two groups were analyzed. Results There was no significant difference in age, weight, MAU and the ratio of hypertension and high cholesterol level between CHD and control groups (P〉0.05). T level in CHD group was significantly lower than that in the control group (P〈0.01), and IMT and LVMI were higher (P〈0.01). Linear correlation analysis showed that T was negatively correlated with MAU, IMT and LVMI in both CHD group (P〈0.01) and control group (P〈0.01). Conclusions Elderly male patients with CHD had a low level of testosterone. T level may be closely correlated with target organ damage.
