摘  要:目的探讨甲泼尼龙及低分子右旋糖酐联合应用对脂肪栓塞综合征的预防效果。方法雄性Wistar大鼠80只,随机分成四组:A组为低分子右旋糖酐组、B组为甲泼尼龙组、C组为甲泼尼龙及低分子右旋糖酐联用组、D组为空白组。四组均采用油酸静脉注射诱发脂肪栓塞综合征,A组、B组、C组在静脉注射油酸前均接受相应药物的预防性使用。观察四组动物各个时间点肺组织病理变化、动脉血氧分压(arterial partial pressure of oxygen,PaO2)、白细胞、肿瘤坏死因子(tumor necrosis factor-α,TNF-α)、髓过氧化物酶(myeloperoxidase,MPO)等指标。结果检查结果显示各用药组对脂肪栓塞综合征都有预防作用,C组效果优于A组、B组;B组效果优于A组。结论甲泼尼龙及低分子右旋糖酐联合应用能阻断脂肪栓塞综合症的某些病理生理环节,可以用于预防脂肪栓塞综合征。Objective To investigate the preventive effect of low-molecular-weight dextran on fat embolism syndrome (FES) and its mechanism.Methods Eighty Wistar rats were randomly divided into four groups: low-molecular-weight dextran group (group A),Methylprednisolone group (B),Methylprednisolone combined with low-molecular-weight dextran group (C) and oleic acid group (D).FES models were established by injection of oleic acid.The pneumono-histopathological change,arterial partial pressure of oxygen (PaO2),leukocytes,tumor necrosis factor-α (TNF-α),and myeloperoxidase (MPO) were examined at different timepoints.Results Preventive effect on FES can be observed in all groups using drugs.The effect was the best in group C,and then in group B.Conclusion Methylprednisolone combined with low-molecular-weight dextran can interfere in some pathophysiological procedures of FES and can be used for prevention.
