常规IVF授精后6h行ICSI挽救IVF受精失败的可行性分析

摘　　要：目的挽救常规IVF受精失败的卵母细胞。方法常规IVF授精后6h观察卵母细胞是否排出第2极体,如果出现第2极体的卵母细胞数少于获得成熟卵母细胞的50%,对未排出第2极体的卵母细胞行ICSI挽救。结果成熟卵母细胞中有69.94%能观察到第2极体的排出,其中有93.91%最终受精。没有观察到第2极体的出现,其中最能获得受精的有13.31%。授精后6h未能观察到第2极体的卵母细胞在立即行ICSI挽救后,受精率、优质胚胎率、胚胎冷冻率、临床妊娠率以及胚胎种植率都较18h挽救组显著提高,与常规ICSI组相似。6h挽救组≥3pn异常受精的比率较常规ICSI略有升高,但无显著性差异,较18h组反而明显降低。结论常规IVF授精后6h,以第2极体是否出现来预测卵母细胞是否能够受精,对预测不能受精的卵母细胞行ICSI挽救能极大的提高卵母细胞的利用率,并获得很好的临床妊娠结局,并且不会带来异常受精风险的增加,这是一条高效、可行的挽救常规IVF受精失败的措施。Objective：To rescue the oocytes unfertilized after conventional IVF.Methods： All mature oocytes were observed on the presence of the second polar body in 6 hours after IVF,rescue ICSI was performed on those with absence of the second polar body.Results： 69.94% of the mature oocytes could be observed the presence of the second polar body,93.91% in which were fertilized in the end.13.31% of the oocytes with absence of the second polar body could be fertilized.Competitive rates of fertilization,good quality embryos,embryos for frozen,clinic pregnancy and implantation were achieved with rescue ICSI in 6 hours after IVF.Conclusions： It was competitive to rescue ICSI on oocytes with fertilization failure in 6 hours after conventional IVF.
