局部瘢痕瓣翻转做衬里修复鼻翼缺损

摘　　要：目的：提高严重面部烧伤后鼻翼缺损的修复水平，减少供区损伤。方法：采用即将被去除的鼻背瘢痕或鼻翼旁瘢痕形成瘢痕瓣翻转做衬里，加上全厚皮移植术修复鼻翼缺损。本组共6例，鼻翼缺损面积较大，均为中重度损伤，而且鼻周围瘢痕组织较多，可选择的手术方法受到一定限制。根据缺损程度及局部条件，分别采用鼻背瘢痕瓣或鼻翼旁瘢痕辩或鼻背加鼻翼旁瘢痕瓣联合翻转形成衬里，加全厚皮移植术，重遣鼻翼，打包加压包扎，硅胶管支撑固定两周，术后10-14天拆线。结果：6例患者，一期愈合者5例；小部分远端皮辩坏死者1例，其残端自行愈合；1例鼻背瘢痕瓣者术后3个月随访，见鼻翼略有回缩，以后情况基本稳定。结论：局部瘢痕瓣翻转做衬里修复鼻翼缺损，方法相对简单，设计合理，损伤小，减少做衬里的程序，远期效果维持良好，是一种值得尝试和推广的鼻翼缺损的修复方法。Objective：After raising the serious face burns, wing of the nose damage repair level, reduces supplies the area damage. This group of altogether 6 exampies, the wing of the nose damage area is big, is the specific weight damage, moreover around the nose the Scar tissue are many, may choose the surgery method receives certain limit. According to the damage degree and the partial condition, use nearby separately the nose back scar petal either the wing of the nose the scar petal or the nose back add nearby the wing of the nose the scar petal union turn over to form the lining, adds the entire thick skin transplant technique, rebuilds the wing of the nose, the pack compression wrap, the silica gel tube support fixed two weeks, the technique latter 10-14 days opens the line. Results：6 example patients, an issue of cicatrization 5 example; The small partial far-end skin petal necrosis 1 example, its remnant end heals voluntarily; 1 example nose back scar petal technique latter 3 month revisit, sees the wing of the nose slightly to have the retraction, later situation basic stable. Conclusion：The partial scar petal turn over makes the lining repair wing of the nose damage, the method is relatively simple, the rational design, the damage is small, reduces makes lining＇s procedure, the forward effect maintenance is good, is one kind is worth attempting and the promoted wing of the nose damage repair method.
