摘 要:目的通过新生儿神经行为测定(NBNA)及脑干听觉诱发电位(BAEP)探讨神经节苷脂治疗中重度新生儿缺氧缺血性脑病的疗效。方法将58例中重度HIE患儿随机分为治疗组(GM1,n=30)和对照组(胞二磷胆碱,n=28)。两组分别在生后第2天、15天、28天进行新生儿神经行为能力测定(NBNA)及脑干听觉诱发电位检测(BAEP)。结果治疗前两组NBNA评分及BAEP异常率均无明显差异(P>0.05);治疗后GM1组NBNA评分高于对照组(P<0.05,P<0.01);BAEP异常率低于对照组(P<0.05,P<0.01)。结论GM1治疗中、重度新生儿H IE疗效理想;NB-NA及BAEP可作为GM1治疗新生儿HIE疗效评价的重要指标。Objective: To evaluate the effect of monosialotetrahexosyl ganglioside (GM1) for moderate and severehypoxic - ischemic encephalopathy neonates by neonatal behavioral neurological assessment (NBNA) and brainstem aditory evoked potentials (BAEP). Methods: Fifty - eight neonates with moderate and severe HIE were randomly divided into GM1 treatment group (30) and control group (n = 28 ). Neonatal behavior and nervous assay (NBNA) and brainstem aditory evoked potentials (BAEP) were performed on the 2th, 15th, 28th day for all infants. Results: No significant difference of NBNA and BAEP were found in HIE neonates with moderate and severe degree between the two groups on the 2th day ( P 〉 0. 05 ) ; NBNA scores in the GM1 group were significantly higher than those in the control on the 5th, 28thday ( P 〈 0.05, P 〈 0. 01 ) ; Brainstem aditory evoked potentials (BAEP) abnormality rates in the GM1 group were significantly lower than those in the control on the 5th, 28th day ( P 〈0.05, P 〈0.01 ). Conclusion: GMI is particularly effective against HIE neonates with moderate and severe degree; NBNA and BAEP can be used as the important indicators of effect for GM1 against HIE.
