摘 要:目的探讨糖皮质激素对胆总管切开T管引流狗模型的T管窦道形成的影响。方法制作狗胆总管切开T管引流模型。分为对照组、糖皮质激素短期使用组、糖皮质激素中长期使用组,每组8只。测量胆总管直径、大网膜长度及有无变异、胃和横结肠的蠕动次数。制模2周后再次开腹,检测T管窦道长度和完整性;以窦道的加压注水量作为窦道所能承受的压力指标,分析不同组间的差异以及多种因素对加压注水量的影响。结果3组之间胆总管直径、胃蠕动次数、横结肠蠕动次数、大网膜长度及窦道长度的差异均无统计学意义(P〉0.05);对照组和糖皮质激素短期使用组的T管窦道形成完整;而糖皮质激素中长期使用组有50%窦道不完整;完整的T管窦道行加压注水,3组间加压注水量的差异有统计学意义(P〈0.01);而在其它的所有因素中对T管窦道加压注水量影响最大的是大网膜长度,两者呈正相关(P〈0.01)。结论中长期应用糖皮质激素明显阻碍T管窦道的形成,影响窦道的完整性和坚固程度;而短期应用对T管窦道形成的影响较小。Objective To investigate the effects of glucocorticoid on the sinus formation of T-tube in dogs with choledochostomy and T-tube drianage. Methods The dogs with T-tube after choledochostomy were divided into the control group, glucocorticoid short-term treatment group and glucocorticoid mid-long-term treatment group, 8 dogs each group. Choledochus diameter, greater omentum length and variation, and the frequency of stomach and transverse colon peristalsis were determined. T-tube sinus length and integrity were detected after 2 weeks, respectively. The influence of different treatment and other factors on the pressurized affusion volume as the pressure parameter of the T-tube sinus solidity was analyzed. Results Choledochus diameter, greater omentum length, and the frequency of stomach and transverse colon peristalsis are no difference obviously in 3 groups (P〉0.05). As compared with the control group and glucocortieoid short-term treatment group, 50% T-tube sinus had not formed completely in glucocorticoid mid-long-term treatment group. The pressurized affusion volume was significantly variation among 3 groups (P=0.002). In other factors, the greater omentum length is the most important factor witch can interfere the pressurized affusion volume and has a positive correlation (P=0.002). Conclusion The glucocorticoid mid-long-term treatment can obviously interfere T-tube sinus formation and influence its integrity and solidity.
