摘  要:目的:探讨产ESBLs菌的发生率、耐药特点,为临床治疗提供参考。方法:采用微量肉汤稀释法对139株大肠埃希菌、64株肺炎克雷伯菌和17株产酸克雷伯菌做药敏试验,同时用纸片扩散法确证产ESBLs菌。结果:产ESBLs菌的检出率分别为:大肠埃希菌41.7%(2007年)和46.8%(2008年);肺炎克雷伯菌25%(2007年)和52.3%(2008年);产酸克雷伯菌66.7%(2007年)和50%(2008年),产ESBLs菌耐药率普遍高于非产ESBLs菌(P<0.01或P<0.05)。结论:常规检测和监测产ESBLs菌十分必要,这对指导临床合理用药、有效控制感染的传播意义重大。Objective : To study the resistant characteristics of ESBLs - producing isolates , and provide references for clinical treatment. Methods : MIC test was taken by broth microdilution method in 139 strains of Escherichia coli ,64 strains of Klebsiella pneumoniae and 17 strains of Klebsiella oxytoca. ESBLs confirmation test was performed for the doubtful isolates. Results :25(41.7% ) of the 60 strains in 2007 and 37(46. 8% ) of the 79 strains in 2008 of Escheriehia coli produced ESBLs. 5(25% ) of the 20 strains in 2007 and 23(52.3% ) of the 44strains in 2008 of Klebsiella pneumoniae produced ESBLs. 2(66.7% ) of the 3 strains in 2007 and 7(50% ) of the 14 strains in 2008 of Klebsiella oxytoca produced ESBLs. The levels of resistance in the ESBLs - positive was significantly higher than those in the ESBLs - negative. Conclusions : It is necessary to examine and monitor ESBLs in normal ,and it is suitable for the treatment and control of infections caused by the ESBLs- positive.
