四川省县级及县级以上公立医院药品收入分析

摘　　要：目的了解四川省县级及县级以上公立医院2003～2007年的药品收入现状,以期对公立医院的改革和补偿机制提供数据支持和参考依据。方法采用文献分析、专家咨询、问卷调查、描述性分析及重复测量资料的两水平模型探讨不同分类医院药品收入占医院总收入比例的差异。结果全省县级及县级以上公立医院的药品收入占医院总收入比例逐年下降;三级医院的药品收入占总收入的比例低于二级医院和其他医院;中药收入占药品收入比例为10.30%;结论2003～2007年,四川省的药品收入占医院总收入的平均比例39.17%低于全国平均水平41.83%;建议政府加大对二级医院和其他级别医院的投入;建议医院通过提高效率、缩短平均住院日、增设药事服务费等方式弥补缺口;加大民族医药在基本药物目录中的比重。Objective To know the drug income of 340 public hospitals ranking country-level and above in Sichuan province from the year 2003~2007,and then provide information for the reform and reimbursement mechanism. Methods We probed into the proportion of drugs in hospital total income by reviewing literatures,consulting experts,collecting questionnaires,and the results were analyzed by statistic description and two-level regression model. Results The proportion of drugs in hospital total income was decreasing year by year; in the hospitals at the level of Grade Ⅲ,the proportion was lower than those of Grade Ⅱ and below; the percentage of traditional Chinese drugs in the overall drugs income was only 10.30%.Conclusion The proportion of drugs 39.1% in hospital total income in Sichuan province is lower than that of national average 41.83%; the government should increase input to hospitals at the level of Grade II and below; the deficit can be maken up by improving efficiency,shortening average length of stay and charging extra fee for pharmacy service; we suggest that the percentage of TCM and folk medicine prescribed from essential drug list be enlarged.
