摘  要:目的:探讨三维增强磁共振血管造影(3D CE-MRA)在颅颈部动脉狭窄诊断中的应用价值。方法:回顾性分析行3D CE-MRA及数字减影血管造影(DSA)检查的颅颈部动脉狭窄患者42例的临床资料。结果:本组3D CE-MRA共发现颅颈部动脉狭窄52处,41处与DSA分级相符,高估11处,其中在颅外段颈动脉狭窄评价方面与DSA取得相当高的一致性,在颅底段、颅内段颈动脉和脑内动脉狭窄评价方面与DSA一致性稍差。结论:3D CE-MRA可作为颅颈部动脉狭窄的有效筛查手段,但在评价颅底、颅内段颈动脉和脑内动脉狭窄方面的准确度不够,不能完全替代DSA检查。Objective: To discuss the application value of three-dimensional contrast-enhanced MR angiography (3D CE-MRA) in cranial carotid artery stenosis (CCAS) diagnose. Methods: The clinical data of 42 patients with CCAS examined by 3D CE-MRA and digital substraction angiography (DSA) were retrospectively analyzed. Results: There were 52 areas of CCAS found in the group by 3D CE-MRA,with 41 areas consistent with DSA classification and 11 areas were overrated, among which extra CCAS evaluation was highly consistent with DSA while cranial basis, intra cranial carotid artery and intracerebral artery stenosis was relative low in consistency. Conclusions:As effective screening means, 3D CE-MRA is not accurate enough on cranial basis, intra cranial carotid artery and intracerebral artery stenosis, which could not completely replace DSA for examination.
