原发性高血压患者尿微量蛋白的改变

摘　　要：对41例Ⅱ、Ⅲ期原发性高血压患者尿微量白蛋白(ALb),球蛋白(IgG)及其清除率进行观察。结果:(1)Ⅱ、Ⅲ期高血压尿ALb、IgG及其清除率与收缩压(SBP)、舒张压(DBP)的单因素间无显著相关。(2)Ⅲ期高血压组尿ALb、IgG及其清除率显著高于对照组及Ⅱ期组;Ⅱ期组尿ALb及其清除率(UAE)也显著高于对照组(P<0.05,P<0.01),但尿中IgG及其清除率正常。(3)Ⅱ期高血压者有16例未经治疗,15,例正在接受ACEI药物治疗,两组之间SBP、DBP之间差别显著,未治疗组尿ALb及UAE高于治疗组(P<0.05,P<0.01)。由此可见,肾功能无明显受损的高血压病人尿中可有微量蛋白,且ALb是比IgG更为敏感的观察早期肾脏改变的指标,应用ACEI治疗可以减少UAE,对高血压病患者的肾脏具有保护作用。Trace proteinuria and clearance of albumin and IgG were determined in 41 patients with essential hypertension in stages Ⅱ and Ⅲ. The results showed: there were no correlations between urinary ALb, IgG, its clearance rates and systolic/diastolic blood pressure. Urinary Alb and its clearance rate in hypertensive patients were significantly higher than normal subjects. Urinary IgG and its clearance rate in stage Ⅲ patients were higher than the stage Ⅱ patients and normal subjects. There were significant differences between patients with and without treatment by antihypertensive drugs(ACEI)in urinary ALb, its excretion rate and blood pressure. In conclusion, the changes of urinary ALb may be an early sensitive indicator of vascular involvement of high blood pressure,and as a marker,urinary ALb excretion rate seems to be much better than IgG.
