氨氯地平治疗原发性高血压合并颈动脉粥样硬化斑块的临床评价

摘　　要：目的评价氨氯地平治疗原发性高血压合并颈动脉粥样硬化斑块的临床疗效。方法选择原发性高血压合并颈动脉粥样硬化斑块患者78例。将患者随机分为2组:治疗组40例口服苯磺酸氨氯地平片5～10mg/d;对照组38例口服马来酸依那普利片5～20mg/d。两组均连续治疗12个月。于治疗前后用高分辨超声技术检测两组患者的颈动脉内膜-中层厚度(IMT),观察两组患者治疗前后血压的变化。结果两组治疗后收缩压和舒张压与治疗前相比均有显著差异(P<0.05),治疗后收缩压和舒张压两组之间相比无显著差异(P>0.05);治疗组治疗后IMT较治疗前显著改善(P<0.05),与对照组比较有显著差异(P<0.05),对照组治疗后IMT较治疗前无显著差异(P>0.05)。结论氨氯地平除具有良好的降压作用外,还具有抗动脉粥样硬化、逆转斑块的作用。Objective To evaluate the therapeutic effect of amlodipine on essential hypertension with carotid artery atheroselerosis. Methods 78 cases of essential hypertension with carotid artery atherosclerosis were randomly divided into two groups, and control group 38 cases. The group of treatment （40 cases） was treated with amlodipine besylate tablets 5 - 10 mg/ d; the group of comparisson （38 cases） was treated with enalapril maleate tablets 5 -20 mg/d. Both groups were treated continuously for 12 months. The carotid artery intima - media thickness （IMT） of two groups of patients were assessed by high - reso- lution Doppler uhrasonography before and after treatment, The blood pressure changes in patients of two groups were observed be- fore and after treatment. Results After treatment, systolic and diastolic blood pressure of two groups compared with those before treatment were significantly different （P 〈0. 01 ） , systolic and diastolic blood pressure after treatment were no significant difference between the two groups （ P 〉 O. 05 ） ; The IMT of the treatment group after treatment improved significantly compared with those before treatment （ P 〈 0. 05 ） , That were significantly different compared with the group of compafisson （ P 〈 0. 05 ）, The IMT of the group of comparisson after treatment had no significant difference compared with those before treatment （ P 〉 0. 05 ）. Conclusion Amlodipine had good antihypertensive effect, but also with anti - atherosclerosis, the role reversal of plaque.
