苯妥英钠对脑卒中患者血清HDL-c水平及卒中复发率的影响

摘　　要：目的观察苯妥英钠对脑卒中患者血清HDL-c水平及脑卒中复发率的影响。方法选择38例脑卒中患者作为苯妥英钠治疗组及同期34例脑卒中患者作为常规治疗对照组,两组常规予以降血压及抗血小板治疗,苯妥英钠治疗组:苯妥英钠0.1 Bid,6个月后改为0.1qd,随访两年,随访期间每6个月检测HDL-c水平,比较两组脑卒中复发率和HDL-c水平变化。结果苯妥英钠治疗组脑卒中复发率明显低于对照组(P<0.05),治疗组六个月后血清HDL-c水平明显升高,与治疗前相比具有显著差异(P<0.01)。对照组血清HDL-c水平与治疗前相比变化不明显(P>0.05)。随访两年,苯妥英钠治疗组6个月后HDL-C仍维持在一较高水平,而对照组与治疗前相比变化无统计学差异。结论苯妥英钠能提高血清HDL-c水平,可以降低脑卒中的复发率。Objective To observe the effects of phenytoin on serum HDL-c in patients with stroke and the relapse rate of stroke. Methods 38 patients with stroke was enrolled in phenytoin group and other 34 patients enrolled in control group. In phenytoin group patients were treated with phenytoin at a dose of 0. 1g , twice a day for six months and after that the dose of phenytoin change to 0. Ig once a day. Patients of two groups were all treated with antihypertensive and antiplatelet. Results The relapse rate of pbenytoin group was obviously lower than that of the control group（P〈0. 05）. The levels of the serum HDL-c in phenytoin group were significantly increased than that before treatment（P〈0. 01） after six months, while the change in control group was not significantly（P〉0. 05）. The levels of the serum HDL-c levels in Phenytoin group maintainm a high level while the control group has no markedly change compare with that before treatment. Conclusion Phenytoin can raise the serum HDL-e content and reduce the relapse rate of stroke.
