铅门在IMRT计划中的应用

摘　　要：目的:在IMRT计划中,在不明显降低计划质量的前提下,使用铅门阻挡部分射野形成局部野,来降低周边OAR剂量体积,尤其是低剂量体积。方法:IMRT计划中,有些角度射野的布置只是因为靶区中的一部分需求所致,而不是整体。这种情况下,利用铅门对该方向的射野进行局部阻挡,形成局部野,只照射需要从该方向照射的部分靶区。这样可以减少该射野的OAR的穿射体积,降低其剂量体积。结果:使用铅门阻挡技术的IMRT治疗计划较之传统IMRT计划可以较好地降低了OAR的剂量体积,尤其是低剂量体积。同时靶区适形指数没有明显下降。结论:铅门在IMRT计划中可以更好地被使用。Objectlve：In IMRT plan, under the premise of not obvious cut down the plan＇s quality,to use blade to block some fieldfor, forming segment field and reduce the OAR＇s dose volume, especially low dose volume.Methods：In IMRT plan,some field places in it＇s angle due to partition of the target, rather than as a whole.Under this condition, we can use blade to block the field, forming segment field, make the field only irradiate the partion target that needing irradiated form this angle.Results：The field blade block＇s IMRT plan can reduce the OAR＇s dose volume, especially lower dose volume, compared with conventional IMRT plan.The target＇s conformal index did not decrease obviously.Conclusion ：The blade can be better used in the IMRT plan.
