摘  要:目的探讨2型糖尿病(T2DM)合并亚临床甲状腺功能减退(SCH)患者血脂及同型半胱氨酸(Hcy)水平的改变。方法比较107例T2DM+SCH组和487例T2DM+甲状腺功能水平正常(EUT)组患者血脂、空腹血糖(FBG)、糖化血红蛋白(HbA1c)及Hcy水平。结果T2DM+SCH组血清总胆固醇(TC)、甘油三酯(TG)及Hcy显著高于T2DM+EUT组(P<0.05)。TC和Hcy与促甲状腺激素(TSH)水平呈正线性相关。结论SCH可加重T2DM患者血脂谱的紊乱程度,进而可能增加糖尿病患者心血管事件的风险。T2DM合并SCH可促使血Hcy水平明显升高,由此进一步促进糖尿病患者发生动脉粥样硬化及血栓性疾病的风险。Objective To investigate the changes of lipids and homocysteine levels in type 2 diabetes mellitus(T2DM) with subclinical hypothyroidism(SCH). Methods Serum total cholesterol (TC), triglycerides (TG), high density lipoprotein cholesterol (HDL-c), low density lipoprotein cholesterol(LDL-c), lipoprotein-α(Lp-α) and homocysteine(Hcy) were compared between the 107 patients with T2DM combined with SCH and the 487 patients with T2DM combined with euthyroidism(EUT). Results TC, TG and Hcy levels were significantly higher in T2DM+ SCH group than those in T2DM+EUT group (P〈0.05). TC and Hcy levels were linearly and positively correlated to TSH level. Conclusion SCH may augment the disturbance of lipid profile, which increases the risk factor for the development of atherosclerosis and cardiovascular occlusive diseases in the patients with T2DM.
