摘　　要：文章认为当前我国农村慢性病综合管理过程中忽视了农民的主体参与作用,没有切实结合农村生产生活方式的独有特点,致使实际干预效果有限。但社会资本理论的引入为农村慢性病综合管理模式的建立、完善和创新提供了一个新的解释切入点和应用起点。The thesis thinks that the effect of chronic disease management is limited in rural areas because of the main role of farmers＇ participation being ignored. But social capital theory can be used scientifically to create, improve and innovate the model of chronic disease management in rural areas.
