摘 要:目的探讨干扰素(IFN)治疗儿童病毒性心肌炎(VMC)的临床效果。方法50例病毒性心肌炎患儿随机分为观察组25例和对照组25例,对照组给予常规治疗,观察组在对照组治疗的基础上加用干扰素。观察治疗前后疗效及心肌肌酸激酶同工酶(CK—MB)、白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α、γ干扰素(IFN-γ)、T细胞亚群的变化。结果观察组总有效23例(92%),明显高于对照组的20例(80%)(x^2=3.87,P〈0.05);观察组治疗后,心律失常恢复正常和S—T段、T波恢复分别为21例(84%)、22例(88%),高于对照组的15例(60%)、16例(64%)(x^2=4.80,x^2=4.87,P〈0.05);两组治疗后IL-6、CK—MB和TNF-α水平高于治疗前(t=2.75,t=2.82,t=2:83,t=2.72,t=282,t=2.88,均P〈0.05);观察组治疗后IL-6、CK—MB、TNF-α和IFN-γ水平高于对照组(t=2.45,t=2.53,t=2.59,t=2.57,均P〈0.05);两组患儿外周血CD3^+和CD4^+含量、CD4^+/CD8^+比值增高。结论干扰素治疗儿童病毒性心肌炎效果较好,且可改善机体的免疫状态。Objective To investigate the clinical effects in children with viral myocarditis(VMC) by interferon treatment. Methods 50 children with VMC were randomly divided into experimental group 25 cases and the conventional group 25 cases;the conventional group was given conventional treatment, the experimental group in the conventional group on the basis plus interferon. The curative effect ,myocardial creatine kinase isoenzyme (CK-MB) ,inter- leukin(IL) -6 ,tumor necrosis factor(TNF) -α,interferon-γ( IFN -γ) and T-cell subsets were observed before treatment and after treatment. Results In the experimental group was effective in 23 cases(92% ) ,significantly higher than the conventional group 20 cases( 80% ) ( x^2 = 3.87 ,P 〈 0. 05 ) ;The experimental group 'after treatment and the arrhythmia back to normal and ST segment .T-wave recovery were 21 cases(84% ) ,22 cases( 88% )were higher than the conventional group 15 cases(60% ) .16 cases ( 64% ) ( x^2 = 4. 80, x^2 = 4. 87, P 〈 0. 05 ) ; The levels of IL-6,CK-MB and TNF-α after treatment were higher than before treatment in two groups ( t = 2.75, t = 2. 82,t = 2. 83,t = 2.72, t = 282, t =2. 88,all P 〈0.05) ;The levels of IL -6,CK-MB,TNF-α and IFN-γ in experimental group after treatment,higber than the conventional group ( t = 2.45, t = 2.53, t = 2.59, t = 2. 57, all P 〈 0.05 ) ;The contents of CD3^+ and CD4^+ were increased,the ratios of CD4^+/ CD8^+ was increased. Conclusion Interferon therapy in children with VMC have good effect and can improve the body's immune function.
