普通单极汽化电切镜在经尿道前列腺剜除术中的临床应用

摘　　要：目的:探讨普通单极汽化电切镜在经尿道前列腺剜除术(TUERP)中的应用价值。方法:应用普通单极汽化电切镜行TUERP治疗良性前列腺增生(BPH)患者25例。对术中出血量、手术时间、术中并发症及术前术后国际前列腺症状评分(IPSS)、生活质量评分(QOL)、剩余尿量(PVR)、最大尿流率(Qmax)、并发症等进行观察分析。结果:25例手术均顺利。手术时间(65±30)min,术中出血(95±60)mL,术后3～5 d拔除尿管。术后无膀胱颈挛缩、尿道狭窄、再次出血,亦无真性尿失禁发生。15例患者术后3个月IPSS、QOL、PVR、Qmax与术前比较均有显著性差异(P<0.05)。结论:运用普通单极汽化电切镜行TUERP对于有经验的腔内泌尿外科医师而言是可行的,无需升级已有设备,可在合适的医疗单位推广应用。Objective： To approach clinical application of monoploar electrovaperization system in transurethral enucleative resection of prostate.Method： All of 25 cases of BPH were treated by transurethral enucleation prostate using monoploar electrovaperization system.The indices such as IPSS,QMax,PVR,QOL were monitored and analyzed at the preoperative and postoperative.Results： The operations were all successful in 25 cases.The operative time was（65±30）min.The blood loss volume was（95±60）ml.Urethral catheter was removed 3～5 days after surgery.After 3 months postoperative the IPSS,QOL,PVR and Qmax were obviously improved after the procedures（P0.05）.Conclusion： The monoploar electrovaperization system in transurethral enucleative resection of prostate is a feasible procedure for experienced doctor,and the procedure should be popularized in suitable hospitals.
