摘 要:目的总结自我管理教育在肝移植患者康复过程中的作用。方法对45例肝移植术后患者进行自我管理教育,并采用疾病知识调查表及症状自评量表对患者进行了测评。结果通过自我管理教育,提高了患者疾病相关知识的知、信、行水平,增加自我保健意识,调动了患者及其亲属积极参与及学习自我管理方法的主动性,有效改善了肝移植术后出院患者的身心健康,使患者及其家属认识到患者是出院后治疗、护理任务的主要承担者,而医护人员的作用是协助他们做到有效的自我管理。结论自我管理在设计上符合患者的健康需求,实施起来简单易行,所需费用低,有较好的成本效益,可以由患者在接受训练后自己组织实施,易于普及推广。Objective To summarize the application of self-management education in the rehabilitation process of liver transplant patients after discharge. Method The content and implementation procedure of self-management education in 45 patients after liver transplantation were described. The patients were evaluated with a questionnaire about disease-related knowledge and the Symptom Checklist-90 (SCL-90). Results The self-management education improved the patients' disease-related knowledge,attitude and behavior,enhanced their self-care awareness,motivated the patients' and relatives' initiative of participating and learning the skills of self-management,and thereby effectively improved the patients' physical and mental health after discharge. Meanwhile,the self-management education made the patients and relatives realize that they were the main bearer of medical treatment and care,and the medical staff 's role was to help them carry out effective self-management. Conclusions The design of self-management education can meet the demands of patients in health. It is easy to put in practice with low cost and better effectiveness,which can be implemented by the patients themselves after training,and is deserved to be carried out in liver transplant patients.
