乳癌彩超血流信号与Survivin表达及淋巴结转移的关系

摘　　要：目的:探讨乳癌血流信号表现与Survivin的表达以及腋淋巴结转移的关系。方法:82例乳癌患者术前采用彩色多普勒血流显像技术(CDFI)检测乳癌血流信号,并将其分为0～3级。0～1级归为不丰富组,2～3级为丰富组。术后采用免疫组化方法检测癌组织中Survivin蛋白表达。以20例纤维腺瘤作为对照组。结果:CDFI2～3级者腋淋巴结转移率为66.7%(24/36),高于0～1级者的腋淋巴结转移率32.6%(15/46)(χ2=9.392,P=0.002)。Survivin蛋白在乳癌组织中阳性表达率为50%(41/82),纤维腺瘤组织中阳性表达率为10%(2/20),差异有统计学意义(χ2=8.973,P=0.002)。Survivin蛋白的表达与患者年龄、病理组织类型及临床分期无关(χ2分别为0.781、1.138及3.169,P>0.05),但与腋淋巴结转移有关(χ2=5.916,P=0.015)。Survivin蛋白在CDFI2～3级者的阳性表达率为66.7%(24/36),高于0～1级者的36.96%(17/46)(χ2=7.13,P=0.008)。结论:乳癌肿瘤内部血流分布与Survivin表达相关,并且与腋淋巴结转移关系密切。Aim：To study the relationship among tumor angiogenesis,the expression of Survivin and axillary lymph node metastasis in breast carcinoma tissue.Methods：Color Doppler flow imaging（CDFI） of 82 cases of breast carcinoma was performed to evaluate the tumor vascular pattern,which were divided into four grades（0~3 grade） according to the CDFI grade.The expression of Survivin was assessed immunohistochemically after operation,compared with 20 cases of fibroadenoma.The association among tumor vascular pattern,the expression of Survivin and lymph node metastasis was studied respectively.Results：Metastatic rate of axillary lymph node in CDFI 2~3 grade was 66.7%（24/36）,higher than that [32.6%（15/46）] in 0~1 grade.Blood flow signals manifested by CDFI was related with the axillary lymph node metastasis（χ2=9.392,P=0.002）.The expression rate of Survivin in breast carcinoma tissue was 50%（41/82）,higher than that [10%（2/20）] in fibroadenoma tissue（χ2=8.973,P=0.002）.The expression of Survivin in breast carcinoma tissue had no relationship with age,pathologic type,or clinical stage of patients（χ2 was 0.781,1.138,and 3.169,P0.05）,but it had relationship with axillary lymph node metastasis（χ2=5.916,P=0.015）.The expression rate of Survivin in carcinoma tissue with CDFI 2~3 grade was 66.7%（24/36）,higher than that [36.96%（17/46）] in carcinoma tissue with CDFI 0~1 grade（χ2=7.13,P=0.008）.Conclusion：Blood flow within the tumor is in close relationship with the expression of Survivin and axillary lymph node metastasis of breast carcinoma.
