摘 要:目的探讨血清瘦素、IL-13、嗜酸粒细胞趋化因子(Eotaxin)及总IgE(TIgE)在支气管哮喘(哮喘)发病中的作用及其关系。方法选择30例哮喘儿童(哮喘组)及22例健康儿童(健康对照组)为研究对象。二组年龄、性别、BMI比较差异均无统计学意义。ELISA法检测各组儿童血清IL-13、Eotaxin,放射免疫法检测各组儿童血清瘦素,荧光酶联免疫法检测其血清TIgE水平。哮喘患儿同时行肺功能检测。结果1.哮喘患儿急性期血清瘦素水平与健康对照组比较差异有统计学意义(P<0.05);哮喘患儿急性期血清瘦素水平与体质量指数(BMI)无明显相关性(r=0.330,P=0.075),哮喘患儿缓解期血清瘦素水平与BMI有明显相关性(r=0.565,P=0.001),健康对照组血清瘦素水平与BMI有明显相关性(r=0.732,P=0.000)。2.哮喘急性期与缓解期、哮喘缓解期与健康对照组之间血清Eotaxin、IL-13、TIgE水平差异均有统计学意义(Pa<0.001)。3.在哮喘急性期、缓解期血清Eotaxin与IL-13、Eotaxin与TIgE及IL-13与TIgE之间均呈正相关;血清瘦素与Eotaxin、IL-13及TIgE之间均无相关性。4.哮喘急性期、缓解期血清Eotaxin与一秒钟用力呼气量(FEV1)及呼气高峰流量(PEF)、血清IL-13与FEV1及PEF之间均呈负相关,血清瘦素与FEV1及PEF、血清TIgE与FEV1及PEF之间均无明显相关性。结论血清TIgE、IL-13及Eotaxin在哮喘发病中彼此相互影响,共同参与哮喘的病理生理改变,血清IL-13、Eotaxin可作为评判哮喘病情严重程度的一项客观指标。瘦素可能参与哮喘的发病。Objective To explore the nosogenesis and the relationship of each other of serum levels of Leptin, IL - 13, Eotaxin and total IgE(TIgE) in asthmatic children. Methods Thirty asthmatic children (study group) and 22 healthy ehildren (healthy control group) were selected. There was no significant difference in general situation between study group and healthy control group. Serum IL - 13 and Eotaxin were measured by enzyme -linked immunosorbent assay while serum Leptin was measured by radioimmunity,and serum TIgE was measured by fluorometric enzyme immunoassay,and the pulmonary function of patients in study group was detected at the same time. Results 1. The levels of serum Leptin of asthmatic children at attack period was significantly higher than that of healthy eontrol group, and the difference had statistical significance(P 〈 0. 05 ) ; the levels of serum Leptin siginifieantly correlated with body mass index in asthmatic children at remission period ( r = 0. 565 ,P = 0. 001 ) and healthy control group( r = 0. 732, P = 0. 000) ; there was no siginificant correlation during the children's attack period( r = 0. 330, P = 0. 075 ). 2. The levels of serum IL - 13, Eolaxin and TIgE of asthmatic children at attack period were significantly higher than those during the remission period and those in healthy control group, and the differences were significant (P 〈 0. 001 ). 3. In asthmatic ehildren, the serum levels of Eotaxin with IL - 13, Eotaxin with TIgE and IL - 13 with TlgE were positively correlation;the serum levels of Leptin had no correlation with Eotaxin,IL - 13 and TIgE. 4. The serum levels of Eotaxin and IL - 13 had negative correlation with tbrced expiratory volume in one second( FEVI ) and peak expiratory flow(PEF) ;the serum levels of TIgE and Leptin were not correlated with FEV1 and PEF. Conclusions The serum levels of IL - 13, Eotaxin and TIgE in asthmatic children had interaction and participated jointly in the nosogenesis. The serum levels of IL - 13 and Eot
