复方苦参注射液联合博莱霉素治疗恶性胸腔积液临床观察

摘　　要：目的:观察复方苦参注射液联合博莱霉素治疗恶性胸腔积液的疗效和不良反应。方法:60例恶性胸腔积液患者随机分为实验组、对照组。实验组:每天静滴复方苦参注射液20 mL,共4周,并每周向胸腔内注入博莱霉素40 mg.(m2)-1及复方苦参注射液30 mL,共2~4次;对照组:每周胸腔内注入博莱霉素40 mg.(m2)-1,共2~4次。1个月后观察两组的疗效、生活质量(KPS评分)及不良反应。结果:复方苦参注射液联合博莱霉素对恶性胸腔积液的有效率为86.7%,明显优于单用博莱霉素组63.3%,差异有统计学意义(P<0.05);实验组治疗后KPS评分上升,与治疗前比较差异有统计学意义(P<0.05),对照组治疗后评分下降,与治疗前比较差异有统计学意义(P<0.05);实验组不良反应与对照组比较差异无统计学意义(P>0.05)。结论:复方苦参注射液联合博莱霉素治疗癌性胸水疗效肯定,能提高患者生活质量,是治疗恶性胸腔积液的有效方法。OBJECTIVE： To observe the efficacy and toxicity of compound matrine infections plus bleomycin in the treatment of patients with malignant pleural effusion.METHODS： 60 patients with malignant pleural effusion were randomized to either experiment group or control group： the patients in the experiment group were intravenously dripped with compound matrine injection（20 mL/per day for four weeks） and their pleural cavity were concomitantly infused with bleomycin 40 mg·（m^2）^（-1） plus compound matrine injection 30ml weekly for a total of two to four times;while the pleural cavity of the control group were infused with bleomycin 40 mg·（m^2）^（-1） per week for two to four weeks alone.At 1 month,the efficacy,quality of life（QOL,KPS score） and side effect were recorded.RESULTS： The response rate（RR） in the experimental group was significantly higher than in the control group（86.7% vs.63.3%,P0.05）;the KPS scores in the experimental group increased significantly after treatment compared with before treatment（P0.05）,while the KPS scores in the control group decreased significantly after treatment（P0.05）;there were no significant differences between the two groups in toxicity（P0.05）.CONCLUSION： Compound matrine injection plus bleomycin showed proved efficacy for patients with malignant plural effusion efficacy in that the QOL of the patients has been improved,thus it serves as an effective regimen for malignant plural effusion.
