摘　　要：目的:探讨复方苦参注射液对慢性乙型肝炎(CHB)患者免疫功能的影响及抗病毒疗效。方法:将72例CHB患者随机分为两组。治疗组36例,用复方苦参注射液20mL静脉滴注;对照组36例,以还原型谷胱甘肽1.2g静脉滴注,疗程6周,治疗前后检测CHB患者血清淋巴细胞因子IFN-γ、IL-2,T细胞亚群及乙肝病毒学指标。结果:复方苦参注射液治疗后可显著升高治疗组血清中IFN-γ、IL-2的水平,与治疗前及对照组对比差异均有显著性(P<0.05);治疗组CD3+、CD4+细胞水平明显上升,CD8+细胞水平明显下降,与治疗前及对照组对比差异均有显著性(P<0.05或P<0.01);HBV复制指标HBeAg、HBV-DNA的转阴率治疗组明显高于对照组,有显著性差异(P<0.01)。结论:复方苦参注射液能提高CHB患者机体细胞免疫功能,抑制乙型肝炎病毒复制,是治疗CHB取得良好前景的药物。OBJECTIVE：To study the effects of Composite Radix Sophora Flavescentis Injection on the immunological function and its anti-viral effect in patients with chronic hepatitis B （CHB）. METHODS： A total of 72 CHB cases were randomly assigned to either treatment group （n=36） treated with Composite Radix Sophora Flavescentis Injection （20 mL,iv gtt） or control group （n=36） with Gluthione （1.2 g,iv gtt） for 6 weeks. The lymphocyte factor IFN-γ,IL-2,T-cell subsets and indicators of hepatitis virology of CHB patients were detected before and after treatment. RESULTS： In the treatment group,Composite Radix Sophora Flavescentis Injection treatment significantly increased the serum levels of IFN-γ and IL-2,and significant differences were noted as compared with before treatment or with control group （P〈0.05）;moreover,serum levels of CD^3＋ and CD4^＋ T cells increased significantly while serum level of CD8^＋ T cells decreased markedly after treatment,and the differences were significant compared with before treatment or with control group （P〈0.05 or P〈0.01）. The negative-conversion rate of HBV replication markers such as HBeAg and HBV-DNA was significantly higher in the treatment group than in the control group （P〈0.01）. CONCLUSION： Composite Radix Sophora Flavescentis Injection can improve cellular immunological function,inhibit hepatitis B virus replication in CHB patients,and which thus enjoys promising future for the treatment of CHB.
